Job Summary and Qualifications

As a Stroke Coordinator you will coordinate all systems related to acute stroke care delivery. In addition to monitoring quality of care issues, and pathways for stroke care management including patient education, you will also act as a clinical liaison between physicians, clinical coordinators and administrative staff. Your primary function in this role is to instruct and educate patients and their family members.

• You will coordinate care of the acute stroke patient (CVA), TIA (transient ischemic attack) patients across the continuum of care by collaborating with appropriate hospital departments and hospital and medical staff.
• Our coordinator researches best practices and clinical practice guidelines, and facilitates their inclusion in development/updates of CVA/TIA patient care protocols.
• You will collaborate with community partners and outreach to ensure community awareness of stroke signs and symptoms through education and health screening opportunities.

What qualifications you will need:

• Associate's degree in nursing required
• 3 years of experience in hospital nursing and 2 years of experience in units providing care to the neurological patient population required
• Experience as a clinical nurse coordinator of a stroke program preferred
