Heluna Health is Hiring a Medical Case Worker II Near Los Angeles, CA

Salary: $24.34-$35.11 per hour

SUMMARY

The Department of Mental Health (DMH) within Los Angeles County is looking for Medical Case Worker IIs to join our Directly Operated Outpatient Clinics. DMH has over 30 outpatient clinics in eight service areas throughout the County. Our clinics provide a spectrum of mental health services to people of all ages to support hope, recovery, and wellbeing.

ESSENTIAL FUNCTIONS

  • Under professional direction, functions as a member of a multidisciplinary team, and provide services to an assigned caseload of clients.
  • Coordinates community resource referrals as needed, board and care, education programs, parenting programs, food and shelter resources, employment resources, etc. Facilitates various groups to assist clients with life skills and recovery.
  • Assists clinical staff with emergency hospitalizations – contacting psychiatric hospitals to arrange a bed and transportation, and monitors client until admitted or ambulance arrives.
  • Makes referrals to and works with various public and private healthcare, mental healthcare, and other agencies such as Department of Public Social Services (DPSS), Department of Children and Family Services (DCFS), Legal Aid, Probation, etc. Assists clients with completing various applications.
  • Interviews patients, family members, and friends, to obtain information about home environment, and family relationships that may have a bearing on the patient's medical and mental health, and treatment outcome.
  • Provides individual and group informational counseling in mental health clinics or specialized mental health treatment programs for patients/clients regarding problems of independent living.
  • Assists homeless consumers with accessing housing resources and working alongside departmental programs and staff to secure housing and other resources (i.e. household goods, etc.). This includes completion/submission of Shelter Plus Care/Section VIII applications.
  • Completes all client related paperwork within the scope of practice in a timely manner (e.g. Progress notes, Tx Plans, et.).

NON-ESSENTIAL FUNCTIONS

  • Participates in team meetings, case conferences, and multidisciplinary team meetings as needed.
  • Other duties as assigned by management team.

 Education/Experience

  • One year of experience as a case manager - OR - A Bachelor's degree from an accredited college or university in a health-related field (such as Social Work, Psychology, Public Health, etc.)

Certificates/Licenses/Clearances

  • A valid California Class C Driver License or the ability to utilize an alternative method of transportation when needed to carry out job-related essential functions.
  • Successful clearance through Los Angeles County’s Live Scan process.
