Helping Hands is looking for a Full Time Registered Nurse to work in our Hospice Division.

Job Description

As a Hospice Registered Nurse (RN) you will be helping patients and their families navigate a very difficult time in their lives. Utilizing hospice best-practice care delivery approaches, you will have all the tools and resources needed to ensure a patient-centered end-of-life experience. As a member of an interdisciplinary team, you will be able to work with experienced teammates who care deeply about delivering a unique experience for every patient.

What you will be doing:

  • Symptom management: Through a thorough assessment you will work closely with the patient’s attending physician and the hospice Medical Director to ensure every patient’s symptoms are well managed.
  • Plan of Care: Evaluate the treatment and medical condition of the patient according to the plan of care. Individualize the plan of care and adjust as needs change.
  • Continuity of Care: Manage interdisciplinary care as applicable while promoting continuity of care with appropriate admissions, transfers, and discharges.
  • Family Educator/Advocate: Counsel patient and family on the disease/progression and incorporate patient and family in the development of the plan of care. Listen to patients and family members.
  • Documentation: Completes all clinical documentation following agency protocol and Medicare/Federal guidelines.
  • Operations: Oversee and supervise total care of patients provided by nurse aides and LPNs. Attend in-service training and mandatory agency meetings.

Skills needed:

  • Exceptional communication and a positive attitude.
  • Patience is a virtue when working with patients, families, physicians, and coworkers.
  • Attention to detail is critical, as is being observant and following directions.

Education:

  • Graduate of an accredited registered nursing program.
  • Bachelor’s degree in Nursing preferred.
  • Certified Hospice and Palliative Nurse (CHPN) certification preferred

Experience:

  • Minimum of 2 to 4 years as a RN with experience in a hospice or home health agency or other related health program.
  • Experience with pain and symptom management interventions preferred.
  • Ability to work within an interdisciplinary setting.

Licensure, Registration, and/or Certification:

  • Must have a current Registered Nurse license in good standing in the State of Pennsylvania in accordance with the Pennsylvania Board of Nursing in accordance with the Pennsylvania Revised Code 4723.
  • Valid driver’s license and reliable transportation that is insured in accordance with Pennsylvania state requirements.
  • Satisfactory references from employers and/or professional peers.
  • Satisfactory criminal background and national sex offender checks.
