The Hellenic Nursing and Rehabilitation Center (HNRC) is a 154 bed, not for profit skilled facility located south of Boston. The HNRC primarily serves the communities of Greater Boston and Southeastern Massachusetts.

Job description

• Reviews all pertinent documentation for accuracy and consistency
• Meets regularly with staff to discuss, educate and enforce documentation requirements
• Assists with division of medical assistance audits
• Assists with the completion of MDS’s as a screening tool, as needed
• Assists with the completion of MDS’s on all residents as needed to establish facility acuity level
• Assists with the scheduling of all MDSs due,
• Assist with Medicare meeting minutes: tracking skilled coverage, days left, level of care changes, 30 day and 60 day windows
• Assists with tracking all MLOA and readmissions, enter into the computer, print and prepare for transmission
• Assists with issuing notices regarding change in status
• Assure all MDSs are signed, dated and transmitted in timely fashion
• Sort and distribute completed MDSs to appropriate units for filing
• Assesses resident wound equipment and supply needs, makes weekly and as needed rounds
• with wound MD and makes recommendations to the Director of Nursing
• Directs and supervises activities of professional and non-professional nursing personnel as it relates to MDS and wounds
• Wound MD recommendations and follow-up
• Directs procurement of supplies and equipment and maintenance of same as it relates to wound care
• Coordinates services rendered by nursing personnel with those rendered by personnel of other departments as related to wound care
• Supports and participates in inservice education programs. Provides instruction and demonstration to staff on units when needed relating to wound care
• Maintains current knowledge of MDS guidelines

Qualifications

• Current Mass RN or LPN License (required)
• Experience with Resident Care Planning and MDS and Medicare/PPS preferred
• Knowledge of federal and state regulations

Pay: $41.00 - $48.00 per hour
