We are hiring RN’s for Full-Time and Part time positions with flex schedules available 

We are currently seeking Registered Nurses for full-time position performing home health care visits to our community of patients. 

Our home care nurses are an essential member of our health care team that provides skilled nursing to patients in their home and collaborates with our therapy team to deliver the best possible care to patients. 

Responsibilities: 

Evaluate and Assess patients using industry based best practices. 

Create care plan in conjunction with Physician to achieve patient's goals. 

Follow up with Physician and properly document doctor's' orders. 

Provide ongoing assessment and treatment while collaborating with interdisciplinary team. 

Monitor clients' and report changes to Clinical Manager and team members 

Visits patients in their home in designated geographic territories 

Case management and collaborate with health care team members 

Accurately document observations, interventions, and evaluations utilizing an Electronic Medical Record. 

Skills Required: 

Ability to work in a one on one setting with the patient in their home. 

Organized with attention to detail. 

Dedicated to providing best patient care. 

Wound care/Wound Vac, Ostomy Care, Venipuncture, Catheter Management, Vital Sign Monitoring, Medication Management 

Qualifications: 

A current license as a Registered Nurse in Virginia. 

A minimum of one year of recent, verifiable clinical (medical/surgical) experience is required. 

Ability to work independently and manage time effectively. 

Ability to travel to cases as assigned
