HeartShare St. Vincent's Services (HSVS) empowers youth and families to overcome the challenges of homelessness, family crises, addiction, mental illness and poverty. Founded in 1869 as St. Vincent's Services in Downtown Brooklyn, we began as a home for poor and friendless boys. In 2014, HSVS was formed through an affiliation with HeartShare Human Services of New York. Through the affiliation, HSVS developed into a multi-program nonprofit agency with four divisions: Foster Care & Preventive Programs, Integrated Health Services, Housing & Residential Services and Youth & Community Programs. Across our agency, staff members serve our clients with compassion, respect and in collaboration with our outside partners. HeartShare is looking for an Intensive QRTP (Qualified Residential Treatment Program) Case Manager.

The QRTP Case Manager will support youths residing in four group homes for youths in foster care. The Intensive QRTP case manager is responsible to ensure compliance with ACS and OCFS requirements for ORTP operations. The QRTP case manager provides case management and social services to high-risk youths and their families and will coordinate intake and discharges to ensure safety and stability. 

Responsibilities:

  • Responsible for intake and discharge coordination; maintain waitlist.
  • Develop relationships with community organizations and resources to improve service access for youths. Coordinate treatment services to ensure continuity of care.
  • Facilitate psychoeducation to the residents and staff.
  • Maintain frequent contact with the families and guardians; arrange visits with families and/or siblings; provide services to the youth and family; and prepare them for reunification where feasible. 
  • Provide counseling and crisis intervention to all residents. Timely follow-up on incidents.
  • Work closely with high-risk youths residing in four ACS Group homes located in Queens and Staten Island.
  • Prepare safety plans as needed in collaboration with the youth.
  • Incident follow ups and reporting to Shared Response 
  • Use Motivational Interviewing (MI) to engage clients through a trauma informed lens.
  • Initiates referrals and provides collaborative services with other service providers such as mental health treatment providers, hospitals, employment services and parole officers.
  • Collaborate with success coaches to ensure youths are receiving appropriate educational support
  • Collaborate with the Intensive Trauma Specialist to support the youths' overall success and well-being.
  • Prioritize achieving permanency with youths and provide support in their transition to an appropriate level of care.
  • Responsible for the documentation of progress notes in a timely manner .
  • Conducts field visits to youth/families, service providers and referral sites as situations require.

Qualifications


  • Master’s Degree in Social Work or a related field preferred;
  • Valid New York State Driver’s License
  • Strong organizational and documentation skills; Strong Verbal, written and advocacy skills
  • Ability to take initiative, be dependable and resourceful, and pay keen attention to detail
  • Ability to handle multiple tasks in a timely manner and in a fast paced, demanding environment
  • Demonstrates the ability to act responsibly in stressful situations in order to undertake emotionally difficult aspects of working.
  • Awareness of the necessity of field work throughout the greater New York area often by public transport, and evidence of a realistic and secure approach to visits.
  • Knowledge and experience leading groups
  • Caseload is flexible based on the needs of the youths and program.
