About HeartShare

HeartShare St. Vincent’s Services is seeking a Family Team Conference Facilitator who will be is responsible for facilitating and tracking all conferences as required by the Administration for Children’s Services utilizing the Family Team Conference Model. The facilitator will ensure the safety and well-being of each child is assessed through a strength-based lens. Utilizing strong engagement skills, the facilitator will yield positive safety and permanency outcomes for children and families with input from a multidisciplinary team and various stakeholders.

Job Summary

Facilitate Family Team Conferences and Service Termination Conferences; maintain fidelity to the Family Team Conference Model and the Family Treatment & Rehabilitation model

  • Utilize consensus decision making process to ensure sound decision-making for children, families, service providers, and community resources.

  • Follow Family Team Conference schedules and communicate with appropriate Administration for Children’s Services and program staff. Hold pre-conferences and debriefings; Schedule and coordinate with Family Services Plan and Administration for Children’s Services as needed.

  • Collaborate and strengthen partnerships with community representatives to ensure participation and involvement in conferences.

  • Generate Family Team Conference/Family Assessment Service Plan Aggregated Summary reports to track and distribute to program management to ensure all conference and Family Assesment Service Plan are completed timely.

  • Complete administrative responsibilities, including the timely documentation and date entry of family team conferences in promis and connections.

  • Facilitate up to three conferences per day which may be in a family’s home.

  • Participate in Clinical Diagnostic Team meetings, individual and group supervision, staff meetings, training and planning sessions as appropriate.

In addition to responsibilities above, performs all other duties as assigned.

  • Remains flexible and adapts to change.
  • Serves as role model for program participants.
  • Conducts self in a professional manner at all times.
  • Remains sensitive and responsive to cultural differences of program participants and staff.
  • Participates in in-service trainings and on-site workshops as required for professional growth and development. Attends external trainings and workshops at direction of management.
  • Represents the organization both within and outside of the work environment in a manner that promotes the mission, vision and values of the agency. Adheres to organizational mission, vision and values, in addition to all other agency policies and procedures.

#INDHP

  • Bachelor / Master's degree or equivalent
  • Child Welfare and Solution Based Casework Experience a plus.
  • Excellent interpersonal skills, strong verbal, and writing skills.
  • Flexibility and ability to work under pressure and meet deadlines.
  • Computer literate (connections and promis proficiency a plus).
  • Two years of Family Team Conference or similar experience in child welfare is preferred
  • Exceptional mediation skills to effectively facilitate group conference situations.
  • Strong clinical skills and strength based perspective.
  • Excellent organizational skills and the ability to handle multiple tasks at one time.
  • Knowledge of core principles and standards for nyc child welfare system.
  • Ability to hold conferences in family’s homes as needed
  • Group skills training and experience a plus.
  • Bilingual (Spanish/English) a plus.
  • Excellent writing, communication and organizational skills.
