HEARTSHARE HUMAN SERVICES OF NEW YORK is Hiring a Day Services Supervisor Near Woodside, NY

The Day Services Supervisor is responsible for the supervision of assigned Senior Direct Support Professionals. It is the Day Services Supervisor’s responsibility to ensure overall quality, effectiveness and timeliness in service delivery and to serve as the leader of the individual program participant’s Program Planning Team. In addition, the Supervisor maintains the day services staff’s focus on maximizing both habilitation services and personalized community inclusion opportunities, as well as providing for the program participants’ overall safety and well-being.

Service Delivery & Support Related to Program Participants

1. Overall coordination and follow-up of program participants’ needs and goals. Ongoing observation of activities, both in the group room and in the community. Maintaining ongoing direct contact with all assigned program participants to ensure comprehensive knowledge of their behavioral, social and functional needs.

2. Planning, organizing and ensuring the implementation of goal-related and valued outcome-related activities, both at the program center and in the community.

3. Reviewing, correcting and signing off on all goals/valued outcomes and community inclusion activities developed and written by program staff.

4. Monitoring and facilitating the inclusion of program participants in their community, according to individual needs, interests, and desires. Monitoring and facilitating professional and social relationships between individual program participants and community members.

5. Serving as a liaison with family members, guardians and outside agencies regarding all needs of assigned program participants.

6. Prior to admission, writing initial Staff Action Plans; thereafter, oversee the updating of these plans on both an annual and semi-annual basis, with input from the program participants, family members, day services team members, and Care Coordinators as required.

7. Serving as team leader at the Annual and Semi-Annual review meetings.

8. Reviewing and certifying the Staff Action plans for each individual receiving services.

9. Review and certify all monthly summaries ensuring that they are completed on time

10. Ensure all staff have documented service delivery in Pcare each day.

11. Reviewing, as needed each assigned program participants’ program plan to determine if the Program Planning Team needs to meet to discuss modifications to existing goals/valued outcomes or implementation of new ones.

12. Overseeing the appropriate and timely collection of services information, including behavioral data, goal-related data, community inclusion logs, and communication books, ensuring their accuracy and completeness.

13. Ensuring that assigned program participants’ records are accurate and up-to-date at all times and that all documents have been uploaded into Pcare.

14. Supervising Senior Direct Support Professionals and Direct Support Professionals in implementing agreed upon valued outcomes and habilitation services and in monitoring program staff in training/teaching individual program participants. Ensuring that supports are provided with consideration for each person’s individualized needs and routine.

15. Supervising group room staff and providing overall supervision of group room activities. Ensuring adequate coverage when group room staff is absent or in an emergency situation.

16. Coordinating any special services that individual program participants may need, including occupational therapy, physical therapy, and speech and language services, as well as other routine and specialized services. Keeping appropriate clinical staff advised of program participants’ needs, goals and services.

17. Following all agency, state and federal regulations to maintain program compliance. Completing all written reports and documentation on a timely basis.

18. Ensuring that program participants are clean and safe at all times and are provided with an appropriate program environment. Ensuring that no program participant is inappropriately left alone or unsupervised.

19. Maintaining confidentiality of program participants at all times.

20. Reporting all incidents, including allegations of abuse and neglect and serious incidents, to either Program Coordinator or Assistant Program Coordinator immediately upon discovery and ensuring that the Justice Center Hotline is called as appropriate. Assist in filing 147 incident reports as necessary.

21. Ensuring that appropriate equipment and sufficient supplies are available for program participant activities and individual needs.

22. Participating in bussing, lunchtime meals, toileting, behavior management and goal work, as necessary and assigned.

23. Providing crisis intervention and counseling for program participants, as required.

24. Serving as an advocate on behalf of program participants.

Staff Hiring, Supervision and Training

25. Participating in the interviewing, hiring and initial in-service orientation of Senior Direct Support Professionals and Direct Support Professionals. Providing ongoing and timely supervision of assigned Senior Direct Support Professionals, including individual monthly staff supervision, quarterly observations and annual Evaluations are completed in sales force. Complete probationary evaluations within identified timeframe for new staff.

26. Providing in-service training for staff as required, including training in writing and implementing goals as well as other areas deemed appropriate by either the Program Coordinator or Assistant Program Coordinator. Ensuring that assigned staff adheres to agency mission vision, values, policies and procedures.

27. Participating in in-service trainings and on-site workshops as required for professional growth and development. Attending external trainings, workshops and/or Program Planning Team meetings at direction of direct supervisor or program management. Maintaining certification in CPR/First Aid

Qualifications

B.A. in human services or related field.
Minimum one year experience working with individuals with developmental disabilities.
QIDP qualifications required.
