Overview

We are looking for a Patient Care Coordinator to join our team. This position reports to the Director Professional Services and will be responsible for the workflow coordination within Home Care Home Base often working weekdays, weekends or after hours. In addition, Patient Care Coordinator must be able to do an assessment and evaluation of patients. This role also assures compliance with agency standards and state and federal laws.

About You

• Currently licensed as a Registered Nurse (RN) in state which he/she will practice.
• A minimum of one year experience in providing medical, oncology, home health care, or hospice nursing preferred, may be specified by state.
• Previous management or supervisory experience preferred.
• Ability to develop and maintain rapport with caregiver and patients.
• Experience in crisis family intervention.
• Demonstrated experience in working with patients and caregivers coping with terminal illness and death.
• Excellent communication skills, both verbal and written. Excellent interpersonal skills.
• Demonstrated problem solving and decision making abilities.
• Ability to travel 25%
