General Description and Purpose
The individual is responsible for providing direct services to the consumer. ILS services can vary in location, such as consumers' homes, care provider's homes, or out in the community. The ILS Service Specialist works under the supervision of the assigned case manager.
Role and Responsibility
1.) Provide 1:1 support and training to assigned consumers.
2.) Provide individual training in conjunction with the service plan set up by the case manager.
3.) Provide complete timesheets, progress reports, and quarterly reports on time.
4.) Maintain positive communication with consumers, family members, employers, and landlords.
5.) Assist with continuous assessments of assigned consumers.
6.) Ensure a safe environment for consumers.
7.) Assist with the development and implementation of a consumer service plan.
8.) Ensure compliance with Consumer Rights.
9.) Handles consumer concerns in an effective/timely manner.
10.) Attends in-service training, maintains current American Red Cross CPR and First Aid
11.) Embodies and demonstrates Heart of the Valley ILS values and philosophies.
12.) Provides thorough and effective communication with other staff and supervisors.
13.) Participates in team meetings and staff meetings.
14.) Complies with policies and procedures.
15.) Completes other duties assigned by the case manager.
Desirable Skills/Education
Must possess a high school diploma or GED. CPR/First Aid Certified or must be completed before working with consumers. Must complete the Orientation period within 30 days. One year of working with the developmentally disabled community is preferred but not required.
Work Schedule
Usual hours are scheduled Monday through Friday (Business Hours). If necessary, it will be required to work some evenings and weekends. The workweek will also depend on the needs of the consumer. The work schedule will be based on a full-time or part-time basis, depending on the needs of the consumer. No holiday or overtime without pre-approval from the Chief Executive Officer.
Other Duties
Requires extensive travel. The position requires transporting consumers to various sites. On-site visits will be made to the consumer's home, place of employment, and out into the community. Must have reliable transportation, a valid California driver's license, with up-to-date car insurance.
Physical Requirements
The candidate must be able to bend, walk, twist, stand, sit, lift, and carry approximately 15 pounds. The candidate must also be able to push and pull 30 lbs.
ILS Service Specialist will report to the assigned case manager.
Job Types: Full-time, Part-time, Temporary
Pay: $16.00 - $23.00 per hour
Expected hours: No more than 40 per week
