Heart of Texas Behavioral Heath Network... is Hiring a YHDP Drop In Case Manager I/11 248 Near Waco, TX

Youth Homelessness Demonstration Program Case Managers for Klaras Center for Families are responsible for linking and connecting young adults in need of homelessness support programming throughout the six-county catchment area with a focus on those young adults served at the DOBEY Drop-In Center.
EXAMPLES OF WORK PERFORMED::

  • Works with young adults (18-25) and their families in mental health, home, community and/or office settings
  • Provides case management, skills training and crisis intervention for young adults
  • Provides and coordinates life skills training to YHDP DOBEY Drop-In Center clients, and works closely with the Navigation Team and other YHDP providers
  • Coordinates Drop-In Center programming, routinely working to secure appropriate community partners with the goal of providing a robust array homelessness support services for young adults
  • Connects to the Heart of Texas Homeless Coalition’s Heart to Home Coordinated Entry system while also performing other necessary Coordinated Entry functions
  • Complete all documentation in an accurate and timely manner as shown on management reports.
  • Refers families of consumers to appropriate social welfare and health agencies to obtain assistance as needed
  • Performs other related duties as required

Knowledge, Skills, and Essential Functions

  • Excellent written communication and computer keyboarding for inputting clinical documentation
  • Excellent verbal communication with consumers, staff, and community providers
  • Time management required to meet service goals
  • Local travel 30% to consumer homes, school, and other community settings
  • Stooping, bending, sitting, standing, keyboarding for prolonged periods, and lifting 15 lbs.
  • Ability to be CPR and PMAB certified
  • Valid driver's license, liability insurance, and reliable transportation required. 30% local travel

Education and Experience

  • Bachelor's or Master's in social work, psychology or related field with ability to obtain Qualified Mental Health Professional credentialing Required:
  • 1 year of case management and working with homeless youth and young adults preferred:

Hours: M-F 8AM-5PM with other as needed:

Salary: $45,000-$49,500 for Bachelors level / $48,150-$52,965 for Masters level
