ESSENTIAL FUNCTIONS:

As a Registered Nurse (RN) in Home Health, you will:

  • Provide and direct nursing care to include comprehensive health and psychosocial evaluations to patients at home in compliance with applicable laws, regulations, and agency policies.
  • Coordinate the total plan of care with other healthcare professionals.
  • Maintain continuity of patient care by planning and exchanging information with physicians, agency personnel, patients, families, and community resources within a Point of Care setting to

RESPONSIBILITIES:

  • Regularly evaluate and re-evaluate the patient’s condition and nursing need to ensure health promotion and coordination of care.
  • Conduct initial and comprehensive nursing evaluation visits, determine the primary focus of care, and develop the plan of care within state-specific guidelines.
  • Ensure patients meet home health eligibility and medical necessity guidelines as defined by payer sources.
  • Initiate, develop, implement, and revise the plan of care in collaboration with physicians and other healthcare professionals involved in care.
  • Provide clinical services within the scope of practice defined by state laws.
  • Coordinate On-Call for after hours and Weekends coverage as needed
  • Stays informed about changes in the field of nursing and home health care. Share information with appropriate organization personnel.
  • Assists the Nursing Supervisor with assessing the competency of clinical staff to promote quality, continuity, and safety of a client’s care.
  • Assists in the development of organization goals. Administers agency policies and procedures.
  • Supervises, educates, and coaches other nursing personnel.
  • Provide information to be used for statistics, reports, and records for purposes of evaluation and reporting of organization activities.
  • Program management of the Home Health services by proving supervision to nursing and other disciplines.
  • Management of Quality Assurance & Performance Improvement program and provide leadership and direction for all of the committees required in the Home Health program.
  • Promotes home health referrals in the health care community.
  • Provide direction to disciplines in the field to ensure quality of care and a safe environment.

REQUIREMENTS

  • Current Driver’s License and vehicle insurance, with access to a dependable vehicle or public transportation
  • Experience in: Supervisory Role, Home Health, Hospice, Long Term Care, Medicare, reviewed medical charts and patient charts for Medicare guidelines, and starting/managing a program from scratch.
  • Current CPR Certification
  • Current RN License
  • Must be fully vaccinated or willing to complete full vaccination by the date of hire, with proof of vaccination required. Requests for religious or medical exemptions will be considered, as permitted by state law.
  • Preferred: Bachelor’s degree in nursing. Must have graduated from a nursing-accredited program.
  • Preferred: Minimum five (5) years of clinical experience with at least half of that time working in home health or related health care organization. Minimum two to five years of directing and guiding employees to provide quality patient care.
  • Demonstrated ability to supervise and direct staff.
  • Excellent interpersonal skills.
  • Analytic/Critical thinker.
  • Excellent customer service skills and ability to deal tactfully with clients, staff, and community.
  • Treat clients, staff, and the public with courtesy, and respect and present a positive public image.
  • Case management skills and policy development are a plus.
  • Ability to travel daily including weekends, nights, and holidays.
  • Understands state and federal regulatory compliance including, but not limited to OSHA privacy.
