Heart City Health is Hiring a Psychiatric Mental Health Nurse Practitioner Near Elkhart, IN

Position Summary:

The Psychiatric Mental Health Nurse Practitioner will provide counseling and medication evaluation and management to a range of clients having mental illness, including substance abuse disorders. This Nurse Practitioner will address substance abuse addition and other behaviors that create or exacerbate health concerns. The primary concern of this Nurse Practitioner is to guide our patients on their journey to mental wellness.

Essential Functions:

  1. Diagnose patient conditions and their causes, including medication assessment. Develops individual service plans based upon assessment data, and provides therapeutic interventions as necessary including the provision of on-going medication management services.
  2. Treat, prescribe, and refer substance abuse treatment modalities.
  3. Screen patients for physical illness and analyze explanations for psychiatric illness, as well as assess risk factors in patients for mental illness and broader wellness evaluations.
  4. Meet patients in person at both our Simpson and Scenic locations.
  5. Work with our physicians to ensure quality of care for our patients’ physical and mental wellness needs.
  6. Maintains documentation of services in our EMR program in a timely fashion. These records include service plans, progress notes, informed consent, treatment plan reviews, any records needed to meet third-party payor or governmental requirements, and any other documentation as needed.

Knowledge, Skills and Abilities:

  1. Extensive knowledge of the etiology of serious mental illness, developmental or substance abuse disorders, and their treatment by a variety of appropriate therapeutic interventions.
  2. Synthesizes complex or diverse information, collects and researches data, uses intuition and experience to complement data, designs department work-flow and procedures.
  3. Excellent communication skills; active listening as well as written and oral comprehension/communication skills; Gives full attention to what individuals are saying, understands the point being made, asks appropriate questions to gain better knowledge of situation(s) and repeats information to ensure understanding; public speaking skills necessary; bilingual (English/Spanish) language skills helpful
  4. Excellent customer service skills; actively seeks ways to assist internal and external customers within the scope of assigned duties
  5. Good basic mathematical skills; uses a calculator or other means to accurately assist in financial matters
  6. Good computer skills; Outlook, Windows, Microsoft Office applications
  7. Good time management skills; self-evaluates the use of time and understands how others may be affected
  8. Cultural diversity awareness and skills; respects all people regardless of race, nationality or social standing
  9. Ability to work independently (self-motivating) and as a team member
  10. Ability to develop a collaborative therapeutic alliance with individuals and make accurate professional judgments
  11. Ability to build and maintain effective working relationships with co-workers, providers, managers, patients, agency resource personnel and community members in general
  12. Familiarity with local community resources for patients with chronic disease
  13. Knowledge of the health and human services infrastructure, health insurance programs and public coverage options
  14. Problem sensitivity skills; empathetic/understanding
  15. Deductive reasoning and problem-solving skills
  16. Organized and detail-oriented

Education, Experience and Licensure:

  1. College Nursing Degree
  2. Active Indiana Nurse Practitioner license
  3. Computer knowledge required; Windows, Microsoft Office applications and internet savvy
  4. Prior experience in behavioral health that included prescribing psychiatric medications
  5. Prior experience in professional office environment preferred
  6. Active DEA certificate
  7. Active CSR certificate
