We are seeking an on-call Spiritual Care Counselor with a focus on hospice to join our team! The hospice Spiritual Care Counselor serves as a resource to patients and/or families according to the plan of care.

We will achieve this by concentrating on patient and family-centered goals focusing not only on the medical needs but also on providing care for the mental and spiritual needs of our patients and those they hold dear.

Hospice Philosophy:
Hospice care is an interdisciplinary approach designed to meet the needs physical, psychological and spiritual of a person at the end of life with the goal of comfort and not cure. Hospice considers the unit of care to extend beyond the patient and include the family and caregivers as defined by the patient.

Responsibilities:

Conduct a spiritual assessment as part of the comprehensive assessment.

Assist in development and implementation of the patient care plan as it pertains to spiritual care

Participate in interdisciplinary group meetings and development of the patient's plan of care

Work in close collaboration with local clergy when desired by the patient

Provide spiritual support in accordance with on-going wishes and needs of the patient/family in accordance with their religious preferences or definition of spirituality.

Qualifications:

Minimum 2 CPE

Ordained, commissioned, or credentialed according to the practices of an organized religious group or has a degree in theology or from a seminary

Comfortable in an interfaith setting

Able to accept different lifestyles, cultures, beliefs and values

Must be a licensed driver with an insured car

The ability to communicate well, both verbally and in writing

Job Types: Full-time, Part-time

Schedule:

• Evening shift
• On call
• Weekends as needed
