HealthyMinds, LLC is rapidly growing and has multiple openings for Comprehensive Community Services (CCS) Service Facilitators!

The CCS program offers a wide array of psychosocial rehabilitation services to clients across the lifespan who have needs for ongoing, high or low-intensity services resulting from mental health or substance use disorders. These are services and supportive activities that assist CCS clients with mental health and/or substance use conditions to achieve their highest possible level of independent functioning, stability, and independence, and to facilitate recovery. Services provided by this position may include:

• Service Facilitation
• Screening and Assessment
• Service Planning
• Diagnostic Evaluations
• Medication Monitoring
• Physical Health Monitoring
• Individual Skill Development and Enhancement
• Employment-Related Skill Training
• Individual and/or Family Psychoeducation
• Wellness Management and Recovery/Recovery Support Services
• Psychotherapy
• Substance Abuse Treatment

The EXPECTATIONS of a HealthyMinds, LLC CCS Service Facilitator are to:

• Be recovery-focused
• Have knowledge of evidence-based practices for treatment of mental health
• Have experience in community-based mental health and substance abuse assessment, case management, treatment planning and/or crisis resolution
• Communicate effectively (both orally and in writing)
• Work independently and remain organized and efficient while being part of a recovery team
• Possess the appropriate professional certification, training, education, experience, and abilities to carry out their prescribed duties
• Have proficient computer skills in order to generate reports, complete forms, and document case activity

The QUALIFICATIONS to become a HealthyMinds, LLC Service Facilitator are to:

• Have earned a master's degree in a behavioral related health field (bachelor's considered)
• Have access to a reliable vehicle, maintain a current valid WI driver’s license, and maintain insurance on vehicle
• Obtain and maintain CCS certification
