HCHC is a dynamic community health center that offers a health care delivery environment that ensures all members of our community have access to high quality primary care, dental, and behavioral health services. We also provide medication assistance support to ensure our patients have access to the lowest cost for their medications, transportation assistance to and from appointments if needed, community resource connections to close gaps with social determinants of care, integrated care models including targeted chronic disease management support with our pharmacy team and much more. We assist patients in getting access to medicaid as well as other insurance options using trained professional staff who serve in these roles.


Hours of operation: Monday-Friday 7:30AM-6:00PM, Saturdays 8:00AM-12:00PMPharmacy: Monday-Friday 8:30AM-6:00PM
General Description of Role and Duties

A Patient Experience Coordinator at the Healthy Community Health Center is a vital part of providing patient-centered healthcare that is accessible and affordable for all. Our Patient Experience Coordinators are part of a friendly and energetic team of professionals who support the health center in achieving patient centered medical care.

The Patient Experience Coordinator is the first person a patient may interact with over the phone or upon arriving at the health center. That means the position is very important in shaping the patients' first impression.

This role is responsible for answering phone calls, scheduling appointments, collecting new patient information, and welcoming visitors to our clinic. Additional duties include, but are not limited to: checking in patients, verifying demographics; insurance and sliding scale eligibility.

Team members are expected to demonstrate successful professional practices, patient care coordination, ethical accountability, and the preservation of patient and staff confidentiality.

Essential functions of the job include:

  1. Answering phones, locating and organizing patient records, and working collaboratively with all departments within the Health Center
  2. Maintaining patient demographics and insurance records
  3. Answering preliminary non-medical questions for patients
  4. Organizing clinic calendars
  5. Serving as a liaison between medical professionals and patients
  6. Prepare the patients to have a successful interaction with the Health Center

This position is authorized to work in any location recognized as within the scope of the HCHC by HRSA.

· Daylight schedule, Monday through Friday with occasional Saturday mornings (depending on position)

Requirements

Minimum Requirements

  • High school diploma or equivalent
  • Must be able to organize patient information
  • Basic addition and subtraction mathematics skills
  • Proven typing skills
  • Ability to multitask such as: answering phones, greeting patients, entering information in a computer system
  • Possess good interpersonal skills
  • Able to work effectively with co-workers and the public
  • Travel may be required to any of our locations (Stoneport, Elkton, or Parkview)

Preferred Requirements:

  • Fluency in Spanish
  • Fluency in a language other than English
