Compensation:
Contingent on experience along with the relocation bonus dependent on length of stay. You will be
taxed as a 1099 Contractor – you will be responsible for travel and lodging.
Professional Qualifications:
Shall be certified through the American Academy of Nurse Practitioners
Possess a current, unrestricted state medical license from any of the 50 states or Puerto Rico
Maintain continuing education/ certification.
Current Advanced Cardiac Life Support (ACLS) and/or Basic Life Support (BLS) certification.
Nurse Practitioner support to be provided shall include, but are not limited to the following:
The contracted Nurse Practitioner shall interview and examine patients, review past medical history and request and/ or perform diagnostic tests and examinations as necessary to make diagnosis for primary care services. They make diagnoses after evaluating information obtained and provide medical or surgical treatment and/ or referral as appropriate.
The Nurse practitioner shall counsel and educate patients, families, community and medical staff about general health care.
The Nurse practitioner shall prescribe drugs and tests necessary to provide quality health care including the use of radiology and laboratory services.
i. The nurse practitioner shall become familiar with and be guided by the formulary and shall follow procedures when prescribing drugs
ii. The nurse practitioner authorized to prescribe pharmaceuticals shall do so according to the availability of drugs listed in the formulary
iii. Pharmacy services will provide instructions to the prescribing contract physician as to substitution of generic drugs for prescribed drugs
iv. The service unit pharmacy must approve drugs not on the formulary
The nurse practitioner shall participate in a continual process of quality of health care evaluation to assure the provision of quality services.

Comply with all facility infection control and safety procedures, practices, and standards
Respect and maintain the basic right of patients, to include but not limited to personal dignity
Referrals- all referrals shall be done utilizing the Patient Referred Care (PRC) process. The PRC Committee utilizing PRC guidelines will review these referrals.
The Contractor shall complete, sign and return the required forms:
Certification of Immunization
Declaration of Employment
Certification of Education and Insurance
Addendum to Declaration of Federal Employment (OF 306) Child Care and Child Care Worker Positions.
The Contractor will complete a federal Credentialing process initiated by the Service Unit. A provider will not be paid or an official representative under this contract until the Credentialing process is full and complete and the applicant has been successfully approved by the Service Unit’s Credentialing Committee. The Contractor shall have no work or health restrictions that would interfere with the performance of patient care services.
