Personal Care Attendant (CNA)
Job Functions

Administrative Functions
1. Assists participants with bathing, grooming, toileting and feeding while in the Center.
2. Accompanies participants to medical appointments and trips to and from the center as needed.
3. Assists with translations services to other departments as needed.
4. Routinely monitors vital signs such as weight, blood pressure and pulse as ordered by the clinic.
5. Maintains personal care supplies such as incontinence supplies, linen, and personal grooming items.
6. Provides laundry service in the Center as agreed upon by IDT.
7. In coordination with the rehabilitation department, lead Transitional exercise programs
8. In coordination with the activities department, assist with leading activities and the walking group.
9. Assists and monitor participants who need supervision while smoking at the center.
10. Maintains a clean and sanitary environment in the Center at all times.
11. Provides needed assistance for participants during meals as identified per IDT.
12. Provides nutritional supplements to participants at the day center as ordered by the Primary Care Physician and / or directed by the Dietitian.
13. Works with the Dietitian to assure that participants are served diet-appropriate meals.
14. Assists in the monitoring of food and fluid intake for specific participants per the participant’s Plan of Care
15. Assists in the monitoring of participant physical and chemical restraints per the participant’s Plan of Care.
16. Assists with ensuring portable meals are sent home with participants as approved by IDT.
17. Assists participants upon arrival in the Center and helps participants to prepare for departure when leaving.
18. Immediately reports any observed changes in a participant’s behavior, health or mobility to the Center Coordinator and Nurse.
19. Participates on the Interdisciplinary Team as required and communicates effectively with participants, family members, caregivers, staff and visitors in the Center.
20. Monitors safety, infection and exposure control, is prepared in case of emergency, and attends to good body mechanics.
21. Completes documentation in the medical record accurately and in a timely manner.
22. Complies with safety policies and procedures, identifying and immediately reporting any potential or actual unsafe acts or conditions to the Center Coordinator and / or Center Manger.
Takes necessary measures to ensure a safety environment for oneself, co-workers, contractors, participants, visitors and others.
23. Consistently meets or exceeds targets for productivity.
24. Continually seeks better ways for delivering services and communicating with participants.
25. Consistently meets or exceeds customer services targets.
26. Demonstrates respect for and promotes participant rights, including dignity, self-determination, access to care, confidentiality and independence.
27. Understands the importance of community involvement and participates as appropriate in activities that link to its communities,
28. Effectively collaborates with staff peers and contractors to meet goals and further success.
29. Complies with all policies and procedures.
30. Demonstrates proficiency in delivering age-specific care.
31. Consistently meets or exceeds quality assessment and performance improvement targets.

Maintains confidentiality of all participant information.
Knocks before entering a participant’s exam room and bathroom.
Ensures the participants’ right to fair and equitable treatment, self-determination, individuality, privacy, property and civil rights, including the right to wage complaints, are well established and maintained at all times.

Communicates well with staff, colleagues, and senior management. Refrains from negative commentary.
Works at maintaining a positive relationship wit other individuals and departments requiring frequent interaction.
Ability to re-prioritize as necessary when confronted with challenges or obstacles.
Always greets staff / family / visitors in a courteous, professional manner responding with empathy when appropriate using positive interpersonal skills.

Education and Experience
TB screening and successful completion of a health examination by a hospital-designated nurse practitioner or physician is required for employment.
