Are you looking for work-life balance? If you answered “yes" to any of these questions – this Home Health Case Manager / Registered Nurse position is the position for you!Daily Responsibilities:
- The Home Health RN plays a vital role in supporting patient care in the home setting and senior living setting while also educating patient and family members/caregivers on how to manage care as well
- Partners with IDT including therapists or counselors to coordinate plan of care and assist with direct patient care
- The RN receives the necessary equipment from the organization to complete day to day care and participate in team meetings in order to resolve concerns and maintain communicat on patient care

- RN License in Michigan
- At least 2 years of RN experience in acute care setting, home care experience is a plus
- Monday-Friday, 8am-5pm
- Permanent, Direct Hire Opportunity
- Rates between $55-$120 per hour based on type of visit
- Average Salary of $75k-$83k per year
- Mileage reimbursement Tuition reimbursement
- 23 days of PTO in your first year Holiday pay
