Field Pediatric RN Case ManagerHealthCare Support is actively seeking aField Pediatric RN Case Manager to fill an opening with a Centene – Superior Health Plan in Amarillo, TX.Daily Responsibilities for Field Pediatric RN Case Manager:

Complete in-home assessment which includes:
SAI Core A to Core Q
ISP
EDP
Other forms as applicable: 1500 forms, SBP, EVV
Assessing the member’s current health status, resource utilization, past and present treatment plan and services, short- and long-term goals, treatment and provider options.
Follow up on requests for items/services
Speaking to providers

Required Qualifications for Field Pediatric RN Case Manager:

At least 2 years of Pediatric experience required
Case management or home health experience
Ability to learn new technologies and enter assessments into program as they’re being conducted
Clear and active TX RN license
Clear and active driving record, reliable transportation, and insurance

Preferred but not a must qualifications for Field RN Case Manager:

Bilingual in Spanish
Case management experience
