Field Patient Care AdvocateHealthCare Support is actively seeking a Field Patient Care Advocate to fill an opening in Pittsburgh Pennsylvania.Daily Responsibilities for Field Patient Care Advocate:

The PCAs will be working with members to ensure care gaps are closed, barriers to care are removed, and improve member and provider experiences.
Telephonic outreach to members
M-F 8:00-5:00 EST

Required Qualifications for Field Patient Care Advocate:

Bachelor's degree or equivalent experience (4 years)
Must be LPN, LMSW, C-SW, LSW, RN in the state of you live in
2 years of experience in patient outreach/health education

Preferred Qualifications for Field Patient Care Advocate:

Bachelor's degree
Project management/lead experience
