We are looking to fill one Chronic Care Management (CCM) nurse position, who will primarily be responsible for delivering high quality clinical care to patients. You must be a residing in the USA (no need to be citizen or permanent resident), must have valid work permit and be licensed in any state.

Our Practice drives efficiency through EHR integration and we seek to further improve through your inputs that will direct productivity and validate the patient experience. Our ideal candidate is a highly motivated person with the work ethic to achieve excellence and provide high quality Patient Care. You will be provided with a scope to expand our CCM programs.

Familiarity and Training in the CCM program is highly preferred. Our CCM team is frequently audited to ensure correct time was spent and documented. Any discrepancies will result in immediate termination of contract. Efficiency and integrity are the key factors.
