JOB SUMMARY:

Under the direction of the Primary Care HIV Prevention Program Director, the Primary Care HIV Prevention Program Outreach Worker is responsible for performing HIV/AIDS and other Sexually Transmitted Diseases (STDs) education and prevention outreach to diverse high-risk people. This position is responsible for performing HIV/AIDS and other STDs education and prevention outreach to sex workers, injection drug users, and the sexual partners of these people, and other high-risk people in their respective communities. HIV/AIDS prevention services include street outreach, risk reduction small group education/support sessions, community-based mobile health screenings, community forums and other innovative interventions. HIV/AIDS prevention services target diverse people, including persons of color, persons who exchange sex, injection and non-injection drug users, youth-at-risk, women at-risk, people with diverse gender preferences, and other diverse populations. Additional responsibilities include: Conducting community testing of and provide counseling to community residents who participate in the program; conducting community engagement activities, educational activities, and meetings with community partners, and representatives within program catchment areas. Staff is knowledgeable about HIV/PrEP, community resources including educational, social and emotional support services, available to clients. Duties may also include recruiting potential clients, administering questionnaires, data gathering, assisting with event organizing, and performing various administrative tasks related to the Primary Care HIV Prevention Program.

ESSENTIAL TASKS:

1. Conduct community testing of and provide counseling to community residents who participate in the program.

2. Perform HIV/AIDS and STD prevention education including safer sex and safer injection drug use based on Harm Reduction Principles.

3. Lead and participate in outreach events at HCC Health Centers, other CBOs and community events.

4. Co-facilitate structured and or unstructured support groups and safer sex workshops for target populations with other outreach workers including administering pre/post knowledge surveys.

5. Disseminate information about HCC's approach to HIV/AIDS, Hepatitis, STDs prevention, education, treatment, and information to improve harm reduction skills; may include distributing bleach, condoms and other related materials to target populations.

6. Achieve monthly Primary Care HIV Prevention Program goals for outreach and testing.

7. Participate in continuing education on best practices for culturally competent, stigma-free testing and HIV/AIDS-prevention service delivery.

8. Serve as a program liaison to other service organizations.

9. Assess program participants for barriers and social determinants of health (SDOH).

10. Collaborate with project staff to address SDOH.

11. Connect identified program participants and eligible HCC patients with HIV treatment providers; this includes providing clients with advocacy and referral assistance.

12. Conduct telephonic outreach to ensure correct use of home test kits, text message support for PREP, etc.

13. Maintain accurate and complete records for tracking of key performance indicators (e.g., number and types of contacts made including number of participants in support groups and workshops, and detailed demographic information or program participants, individuals screened/tested, PrEP prescriptions, etc.) report this information to the Program Director.; and

14. Assure the availability of necessary supplies for weekly field activities.

15. Utilize health record and other tools to communicate with program participants.

16. Immediately report any discovered or suspected violations of HCC’s protected health information privacy or security policies and procedures to the Program Director or designee.

17. Participate in HCC’s continuous quality improvement program.

18. Agree to telework from home as needed (e.g., during a public health emergency).

19. Perform other job-related duties that may be assigned.

PHYSICAL ACTIVITIES AND REQUIREMENTS, VISUAL ACUITY, AND WORKING CONDITIONS OF THE POSITION

1. The physical activity of this position includes:

1.1. Standing: Remaining upright on the feet;

1.2. Walking: Moving about on foot to accomplish tasks, moving from one work area to another;

1.8. Grasping: Applying pressure to a computer mouse and cell phone with the fingers, palm or whole hand;

1.9. Repetitive motions: Making substantial movements (motions) of the wrists, hands, and/or fingers during examinations or inputting data into a computer;

1.10. Talking: Expressing or exchanging ideas by means of the spoken word; those activities where detailed or important spoken instructions must be conveyed to other workers accurately, loudly, or quickly;

1.11. Hearing: Perceiving the nature of sounds at normal speaking levels with or without correction, and having the ability to receive detailed information through oral communication, and making fine discriminations in sound.

2. The physical requirements of this position include sedentary work: Exerting up to 10 pounds of force occasionally and/or a negligible amount of force frequently or constantly to lift, carry, push, pull or otherwise move objects, including the human body. The position involves sitting most of the time. Walking and standing are required only occasionally.

3. Close visual acuity to perform activities such as: Entering data into a computer terminal; viewing a computer terminal; extensive reading.

4. Environmental conditions for this position include:

4.1. Exposure to outside environmental conditions as they affect travel within a community;

4.2. Exposure to internal environmental conditions including protection from outside weather conditions and but possibly not from temperature changes.

4.2. Possible exposure to body fluids, infectious diseases, and hazardous chemicals.

4.3. Possible contact with people with impaired self-control related to psychiatric or intellectual disabilities or co-occurring substance abuse disorders.

MINIMUM QUALIFICATIONS AND EXPERIENCE REQUIREMENTS

1. Undergraduate degree from an accredited college or university in the social services or health care administration; graduate degree preferred.

2. At least two years’ experience serving in a position that included health-related outreach, education, or testing responsibilities.

3. Experience working in an HIV/AIDS-related prevention or treatment program in a community health center setting preferred.

ESSENTIAL KNOWLEDGE, SKILLS AND ABILITIES

1. Knowledge of prostitution, HIV/AIDS and general awareness of the local client base.

2. Knowledge of pre- and post-test testing techniques and counseling.

3. Experience working with people with substance use issues including IV drug users.

4. Cultural sensitivity with willingness to work with people of diverse ethnic backgrounds, sexual orientations and gender preferences, as well as people with, or at risk for, HIV/AIDS.

5. Working knowledge of computer software (e.g., Microsoft Office) and electronic health record systems.

6. Knowledge of Medicaid, Social Security and other entitlements preferred.

7. Demonstrated competency in written, verbal and computational skills related to making presentations and entering documentation into health records in accordance w1th medical record standards.

8. Ability to establish effective working relationships with a variety of people at all levels of the organization and in a community.

9. Ability to work with diverse individuals.

10. Ability to interact with patients, staff, and other stakeholders in a respectful, professional, and tactful manner.

11. Ability to make reasonable and sound evaluative judgments.

12. Ability to use a computing device and electronic health record software.

13. Ability and willingness to regularly travel.
