General Summary

The Medically Tailored Meal Program Coordinator is responsible for coordinating all aspects of The Health Trust’s Medically Tailored Meal Program with an emphasis on careful program monitoring to ensure both customer satisfaction and fiscal responsibility. This position performs complex technical and administrative tasks related to client outreach, recruitment, onboarding, documentation and support. The Program Coordinator works with clients to ensure they receive and benefit from food supports and nutrition counseling, working towards better health outcomes. This position works collaboratively with case managers, social workers, managed health plan partners and others in the community. Duties also include ordering of meals from vendors, tracking payments and supervising student interns.

Duties and Responsibilities

  1. Customer/Client Duties:
  • Coordinates and confirms receipt of client referrals with managed health plans and referring case managers. 
  • Processes incoming referrals, onboards new clients and sets up program services (meal or grocery deliveries) in Salesforce. 
  • Checks and monitors client eligibility for the Medically Tailored Meals program. 
  • Establishes and maintains effective communication with clients and their families/emergency contacts to ensure successful program outcomes. 
  • Enters client information into Salesforce, monitoring service units to ensure that each client receives but does not exceed approved quantities. This includes tracking missed meals and hospitalizations, requesting and updating authorization dates and providing feedback to referrers. 
  • Coordinates and provides phone support with the Food Basket and delivery drivers to troubleshoot issues and monitor needs.
  • Submits client referrals and re-referrals for Community Supports through CalAIM, providing the necessary documentation and motivation.
  • Coordinates with food vendors and the Food Basket to place weekly food orders. Provides feedback to food vendors as needed.
  • Collaborates with the Meals on Wheels team and the Registered dietician to ensure that meals and groceries are delivered in accordance with policies and procedures. 
  • Assists the registered dietician to conduct surveys and determine meal adjustments.
  • Works with our billing partners to ensure accurate medical billing for services rendered. 
  • Collaborates with the Salesforce partners and the Meals on Wheels team to improve processes and resolve problems. 
  • Prepares and tracks letters, documentation and instructional materials for clients. 
  • Provides other client/customer support services as needed.
  • With Manager supervision, oversee student interns including orientation; monitoring and check-ins and evaluation of performance of interns
     
  1. Provides Clerical Duties: 
  • Maintains adequate records and other documentation necessary for the collection of data and statistics pertaining to program outcomes, demographics, and information as required by funders
  • Completes client surveys and participates in the analysis of data. 
  • Answers phones, taking messages and resolving problems.
  • General office duties that include copying, faxing, mailing and word processing. 
  • Updates collateral materials, forms and other literature as assigned.
  1. Represents The Health Trust through participation in community outreach activities.
  2. Actively models and communicates the mission and vision and supports a corporate culture of empowerment, team building, and open communication.
  3. Attends job training and performs other related duties as assigned.
  4. Upholds HIPAA office procedures and confidentiality. 

Experience and Training

  1. Bachelor's Degree preferred or equivalent experience.
  2. Minimum two years of progressively responsible administrative/office environment.
  3. Experience in a community health field is preferred.
  4. Proficient in Microsoft Office products/software and Google applications, experience with Salesforce a plus.
  5. Bilingual in one or more threshold languages (Spanish, Vietnamese, Tagalog, Mandarin, Cantonese, Farsi) strongly preferred

Minimum Knowledge and Ability

  1. Excellent telephone, communication and interpersonal skills.
  2. Ability to maintain productive working relationships with clients, program staff, interns, volunteers, managed health plan staff and community agencies. 
  3. Ability to communicate effectively with diverse populations. Must be culturally sensitive.
  4. Ability to show understanding, patience and tact when dealing with clients, referrers and community partners.
  5. Knowledge of business correspondence and ability to write clearly and effectively and to proofread for grammar, spelling, and punctuation with a high degree of accuracy.
  6. The ability to perform basic math such as addition, subtraction, multiplication, division, and calculating decimals, percentages, and fractions.
  7. Ability to work independently and/or with general supervision.
  8. Ability to prioritize, problem solve, and utilize critical thinking and good judgment skills.
  9. Knowledge of principles of research and data collection.
  10. Aptitude for learning new database software. 
  11. Ability to model and communicate The Health Trust’s mission and support a culture of empowerment and teamwork.

Reporting Relationships

Reports to Program Manager, Food and Nutrition Services

 Pay Rate: $26.26/hr - 28.90/hr
