Health Sciences High and Middle College (HSHMC) is a Charter High School in City Heights, San Diego, CA serving grades 9-12.

Transition Counselor

Job Purpose:

To plan, prepare, implement, and monitor a structured process of supporting students in their transition from secondary school to the community and various post-secondary opportunities.

Education- and Experience-Based Qualifications:

  • Bachelor’s degree from an accredited institution; and
  • Master’s in Rehabilitation Counseling (Necessary to be considered for position)
  • Minimum of 1 year of experience in transition counseling; and
  • CPR Certification

  • Ability to provide high quality instruction in an individual, small/whole group, or co-teaching setting
  • Ability to establish and maintain cooperative and effective working relationships with students, families, community partners, administrators, general educators, educational specialists, and support staff.
  • Ability to communicate effectively orally and in writing.
  • Must have the ability and proven ability to report to work on a regular and punctual basis.
  • Perform all other related work delegated or required to accomplish the objectives of the school culture and focus.
  • Knowledge and implementation of relevant technology within the classroom curriculum and setting.
  • Knowledge of subject matter/course content and resources relevant to assignment
  • Meet professional teacher education requirements of school/district and state.

Job Duties and Responsibilities:

· Support students throughout their transition process from secondary school to the community and various post-secondary opportunities

· Ensure that students graduate HSHMC with an internship experience portfolio and resume.

· Attend and provide input in IEP and planning meetings including developing and presenting transition goals for individual students

· Establish and maintain community and industry partnerships including expanding pathway opportunities

· Collaborate with internal and external personnel for the purpose of implementing and/or maintaining instructional programs and services.

· Plan, prepare, and deliver high-quality instruction in an individual, small/whole group, or co-teaching setting.

· Provide on-the-job experiences and feedback.

· Monitor students’ academic progress and provide appropriate feedback on work.

· Implement accommodations and modifications within transition experiences in collaboration with the educational specialist and support staff.

· Conduct formative and summative assessments of students.

· Implement competency-based grading system consistent with HSHMC policies and agreements.

· Engage in data focused conversations based on research-based best practices in an effort to improve teaching and learning.

· Attend and provide input in IEP and planning meetings.

· Utilize instructional technologies in a blended learning format to facilitate student achievement.

· Communicate with families and parents to discuss academic progress when there are concerns and celebrations.

· Establish and implement classroom and internship norms and behavior management strategies that align with the school culture.

· Maintain accurate and complete records of students' attendance, progress, and development.

· Update all necessary records accurately and completely as required by laws, district policies and school regulations.

· Attend restorative conferences as needed.

Other functions of the job include, but are not limited to the following:

· Demonstrate preparation and skill in working with students from diverse cultural, economic and ability backgrounds

· Encourage parent and community involvement, obtain information for parents when requested, promptly return phone calls and answer emails.

· Participate in appropriate professional learning activities.

· Participate in extracurricular activities such as social activities, sporting activities, clubs and student organizations as directed.

· Other duties as assigned.

Working Conditions (Physical Requirements and Environment):

Regularly required to sit, stand, walk, talk, hear, operate a computer, hand-held learning devices and other office equipment, reach with hands and arms, and must occasionally lift and/or move items.

Internship opportunities frequently occur off-site. This will require staff to ride a bus or use private transportation, maneuver around various community locations, and demonstrate needed skills, as needed.

Job Type: Full-time

Pay: $55,000.00 - $70,000.00 per year
