Prototypes S. Mark Taper Family Living Center is a 32-unit apartment complex that shares a campus with Prototypes Residential Substance Abuse Treatment Program and Prototypes Outpatient Program. The Housing Program at S. Mark Taper Family Living Center serves women and women with children with Substance Abuse and Mental Health Disorders. The Housing Program also services 37 scattered-site clients living around the Pomona area. The Housing Program provides case management services to help clients work toward acquiring permanent housing. Prototypes Housing Program is a fast-paced environment, as we strive to remain on the cutting edge of service provision.
As a member of the Housing Supportive Services Team, under the direct supervision of the Housing Supervisor, the Client Safety Navigator support clients enrolled in the interim programs funded by the Department of Health Services (DHS) and Substance Abuse Prevention and Control (SAPC). These programs serve individuals who have experienced long-term homelessness, history of trauma, and have long-standing health and mental health issues. Client Safety Navigator also supports the clients residing in permanent housing contracts, as needed. Client Safety Navigator provides program supervision for afternoons, evenings, or graveyard shifts as scheduled by Housing Supervisor.
Key Responsibilities
  • Conducts rounds during shifts to ensure units are clean and clients are safe.
  • Reinforces curfew and other program procedures.
  • Supervises clients and children during all assigned shifts.
  • May oversee clients’ independent living skills (cooking, cleaning, meal planning, shopping, etc.).
  • Provides follow-up to staff to ensure clients are moving toward goals as per service support plan.
  • Supports clients when dealing with interpersonal conflicts with others.
  • Conducts/participates in House Meeting to support clients and promote a safe and amicable environment in the units.
  • Encourages clients to be engaged in program and program demands.
  • Engages in conversation with Housing Supervisor for consultation if needed.
  • Medication monitoring and SAM-L sheet creation and filing.
  • Review/file Sign-In Sheets and Visitor Logs.
  • Ensure office is kept clean and organized and bathroom is properly stocked with supplies.
  • Control the inventory for program (linens, food, office items, etc.).
  • Other duties as assigned by Housing Supervisor.
Education and Knowledge, Skills and Abilities
Education Requirements:
  • High School Diploma/GED/or equivalent.
Experience:
  • A minimum of two years’ experience in a substance abuse treatment setting is preferred.
  • Basic understanding of child development, child abuse, substance abuse and self-sufficiency issues.
  • Bilingual English/Spanish is preferred.
  • California Driver’s License with proof of insurance.
  • Experience working with substance abusers, chronically homeless and/or re-entry populations.
  • Experience with efficient and effective treatment and management of clients with co-occurring mental health and substance use disorders.
  • Knowledge of and experience with providing Trauma- Informed Care, Housing First principles and Harm Reduction services.
  • Strong computer skills, including Outlook, Excel, Word and PowerPoint.
Background Clearance Required:
  • CDCR fingerprint clearance.
  • Must not be on parole or probation for a minimum of two years.

In 2015 Lyon-Martin Health Services became the third primary care clinic operated by HealthRIGHT 360, continuing its specialized care for women and transgender individuals. Tenderloin Health Services (formerly Glide Health Services clinic) became the fourth primary care clinic in 2015, providing care in San Franciscos Tenderloin neighborhood. Prototypes joined in 2016 expanding behavioral health care for women and children and services to survivors of domestic violence in Los Angeles, Orange and Ventura Counties. Strengthening services for women and girls in San Francisco, Womens Community Clinic joined our family of programs in 2017.
