Health Plan of San Mateo is Hiring a Health Promotion Program Specialist Near South San Francisco, CA

Under the direction of the Health Promotion Manager, the Health Promotion Program Specialist leads ongoing Health Education Program initiatives, other Health Promotion Unit priorities and functions.
Position overview
  • Assess member and provider health education and cultural and linguistic needs to establish priorities intervention approaches for specific health issues and target populations.
  • Develops and maintains community partnerships to support HPSM’s Health Education Program initiatives.
  • Supports ongoing review and updates of health education policies, and other programmatic documents such as program descriptions, work plans, evaluations.
  • Leads the process for collecting, consolidating, and reporting themes and topics emerging through the health education line.
  • Acts as liaison with the community and other community-based organizations and maintain effective working relationships.
  • Participates in DHCS led and other Health Education related collaboratives.
  • Supports the submission of DHCS required approval forms including Member Incentives, Focus Groups, and Texting Campaigns and other health education related reporting requirements.
  • Identifies priority health problems in HPSM’s member population and coordinates with existing community resources and programs, or assists in development of new programs.
  • Participates in internal and external meetings, collaboratives and coalitions related to health education projects and population health management related projects.
  • Maintains regulatory required documentation to support ongoing audit or regulatory related requests.
Health Education Program:
  • Leads health education interventions and improvement activities.
  • Identifies and evaluates health promotion resources and materials aimed at improving health status or outcomes available to HPSM members. Including hospital or community- based health promotion classes/programs and member health promotion materials.
  • Develops and maintains health promotion communication content including the health tip section of HPSM's website and articles for the member newsletter. Ensure member content is developed at the appropriate literacy level and that they are available in any required or needed languages.
  • Leads health promotion program enrollment processes and associated education activities.
  • Communicates directly with members to assist in accessing appropriate health education activities and tracks targeted member outreach activities and enrollment into Health Promotion Programs (i.e wellness & self-management programs, member incentive programs). Serves as an escalation point for other HPSM member facing staff.
  • Supports community health education activities, including presentations about HPSM’s Health Promotion Programs and HPSM’s presence at community events.
  • Supports members access to linguistically appropriate health care services and Health Promotion Programs.
Requirements
These are the qualifications typically needed to succeed in this position. However, you don’t need to meet every requirement to apply.
Education and experience
  • Three (3) years’ experience working with culturally diverse populations.
  • Must have one of the following qualifications or demonstrates similar experience:
  • Certified Health Education Specialist (CHES) awarded by the National Commission for Health Education Credentialing, Inc.
  • MCHES (Master Certified Health Education Specialist) awarded by the National Commission for Health Education Credentialing, Inc.
  • Master of Public Health (MPH) degree with a specialization in health education or health promotion, from a program of study accredited by the Council on Education for Public Health, sanctioned by the American Public Health Association.
Knowledge of:
  • Personal computers and proficiency in Microsoft Office Suite applications, including Outlook, Word, Excel, Access and PowerPoint.
  • Principles and methods of health education, health promotion and health behavior theories.
  • Health literacy principles and its potential impact on health outcomes
  • Effective teaching methods and curriculum materials.
  • Program/intervention planning methodology.
  • Health education program evaluation methodologies.
Ability to:
  • Work cooperatively with others.
  • Work as part of a team and support team decisions.
  • Communicate effectively, both verbally and in writing.
  • Adapt to changes in requirements/priorities for daily and specialized task.
  • Develop low-literacy health education materials and health care provider materials.
  • Adapt the presentation of material to the group addressed and the media used.
  • Develop and maintain collaborative relationships with others.
  • Function effectively as a team player.
  • Make group presentations.
  • Facilitate workgroup meetings.
  • Utilize project management skills to manage multiple projects.
Other:
  • Bilingual proficiency in and HPSM threshold language highly preferred.
  • Must have own car or reliable transportation, unrestricted California driver’s license and automobile liability insurance in conformity with California state law minimums and acceptable to HPSM. Must be willing to travel in/to San Mateo, San Francisco and Santa Clara counties (primarily) and up to Sacramento on occasion.
Salary and benefits
The starting salary range is $76,232-$97,921 per year, depending on the candidate’s work experience.
