Under the direction of the Health Promotion Manager, the Health Promotion Program Manager is responsible for the program development, implementation, execution and evaluation of programs that fall under the scope of HPSM’s Health Education System. This role also actively contributes to the ongoing development of HPSM’s Population Health Strategy with a specific focus on Health Promotion programs and initiatives.
Position overview
Plan, execute and evaluate program initiatives to improve performance especially as it relates to key components of Population Health Strategy & Health Education System.
Stay informed about industry best practices and identify opportunities to integrate best practices into Health Promotion Programs.
Disseminate program updates, performance data, evaluation findings and recommendations for continuous improvement on a regular basis.
Work collaboratively with internal stakeholders and with external community organizations and agencies to address identified opportunities for enhancement and alignment.
Educate and work with external partners and participating practices as needed to identify innovation opportunities and adopt improvement actions for priority Health Promotion areas.
Work closely with the Health Promotion Manager to facilitate implementation of any regulatory requirements, facilitate cross-departmental workgroups and represents Health Promotion in other cross-departmental meetings and workgroups.
Establish and maintain relationships with local and community partners to align efforts across priority topics.
Facilitate annual updates and ongoing maintenance of key program documents including Program Description, Workplans, and evaluation.
Lead larger-scale specific programs, especially those with a heavy regulatory focus. These include programs such as member wellness, member incentives, chronic conditions self-management, and health promotion communications campaigns. Examples of current programming include efforts related to diabetes management and prevention, perinatal programs, hypertension self-management, obesity/weight management, asthma management and tobacco cessation.
Lead unit programmatic oversight functions across priority areas including vendor/community partner relationship management, program dashboard tracking & data sharing requirements, referral processing, integration of community resources, programmatic communication plans and evaluations.
Responsible for maintaining broader Health Promotion communication campaigns across priority areas such as: tobacco use and cessation; alcohol and drug use; injury prevention; prevention of sexually transmitted diseases, HIV/AIDS, and unintended pregnancy; nutrition, weight control, and physical activity; parenting as well as self-care of health conditions including pregnancy, asthma, diabetes, and hypertension.
Contribute to project teams, assigning and monitoring work of team members and providing guidance and leadership.
Take the lead, or partner with teams, to investigate or to launch Health Promotion related program initiatives.
May act as the Health Promotion liaison to organizational initiatives and projects.
Lead ongoing vendor management activities/meetings for the Health Promotion team as needed.
Secondary Functions:
Professionally represent HPSM at internal and external meetings as directed by the Health Promotion Manager.
Perform other duties as assigned.
Requirements
These are the qualifications typically needed to succeed in this position. However, you don’t need to meet every requirement to apply.
Education and experience
Two (2) years’ experience working in project management/program management or leading Health Promotion/Health Education initiatives, preferably in a health care or public health environment.
Have obtained, or working towards:
Master of Public Health (MPH) degree with a specialization in health education or health promotion OR
MCHES (Master Certified Health Education Specialist) awarded by the National Commission for Health Education Credentialing, Inc.
Knowledge of:
Principles and methods of health education, health promotion and health behavior theories.
Health literacy principles and its potential impact on health outcomes
Effective teaching methods and curriculum materials.
Program/intervention planning methodology.
Health education program evaluation methodologies.
Abilities:
Work cooperatively with others.
Work as part of a team and support team decisions.
Adapt to changes in requirements/priorities for daily and specialized tasks.
Develop low-literacy health education materials.
Adapt the presentation of material to the audience addressed and the media used.
Develop and maintain collaborative relationships with others.
Function effectively as a team player.
Make group presentations.
Facilitate workgroup meetings.
Utilize project management skills to manage multiple projects.
Skills:
Personal computers and proficiency in Microsoft Office Suite applications, including Outlook, Word, Excel, and PowerPoint.
Communicate effectively, both verbally and in writing.
Bilingual proficiency in English plus Spanish, Mandarin, Cantonese, or Tagalog preferred.
Other
Must have own car or reliable transportation, unrestricted California driver’s license and automobile liability insurance in conformity with California state law minimums and acceptable to HPSM.
Must be willing to travel in/to San Mateo, San Francisco, and Santa Clara counties (primarily) and up to Sacramento on occasion.
Salary and benefits
The starting salary range is $92,240.41 - $118,484.96 per year, depending on the candidate’s work experience.
