Job Overview:

We are seeking a compassionate and dedicated Social Worker to join our team. As a Social Worker, you will be responsible for providing support and assistance to our geriatric patients. Your role will involve conducting assessments, developing care plans, and connecting individuals with necessary resources and services. You wil be working in an outpatient medical center interviewing patients and helping them with their different needs. Must be bilingual: English & Spanish

Duties:

- Conduct comprehensive assessments of individuals' needs and develop individualized care plans

- Provide counseling and support to individuals and their families

- Utilize motivational interviewing techniques to empower individuals to achieve their goals

- Collaborate with interdisciplinary teams to coordinate care and ensure continuity of services

- Maintain strict confidentiality in accordance with HIPAA regulations

- Experience with ACCESS processes to help the elderly community with various applications and programs.

- Knowledgeable of programs such as Plan 8 and others to help low income patients.

Skills:

- Strong motivational interviewing skills to engage with older adults.

- Ability to collect, analyze, and interpret data for effective treatment planning

- Experience working with people with low income and understanding their unique needs.

- Excellent patient care skills, including active listening, empathy, and effective communication

- Knowledge of HIPAA regulations to ensure the privacy and confidentiality of patient information

- Ability to remain calm under pressure and provide crisis intervention when necessary
