We are looking for a Nutritionist/Dietician for supporting our members and championing happier, and healthier lives. In this outpatient setting, you can set your own schedule, focus entirely on primary care and spend more time investing in longitudinal relationships as well as thoughtful decision-making through advanced technology and a team-based approach.

Responsibilities:

• Performs a comprehensive assessment by interpreting multiple data points and synthesizing the interrelationships of the data.
• Performs individual dietary analysis, meal planning and menu development
• Formulates an individualized nutrition plan of care based on assessment findings, interpreting trends in patient populations to achieve best practice. Anticipates nutritional intervention issues, monitors patient tolerance to nutrition therapy, and responds when standards of nutrition care recommend changes.
• Coordinates the nutrition care plan with the primary care physician and other health care providers and serves as a resource in the integration of the nutrition care plan into the comprehensive plan of care for the patient. Advocates patient rights from the family/patient perspective. Makes clinical nutrition decisions based on standards of nutrition care, patient population data. Documents all patient nutrition care activities as per documentation standards.
• Communicates with the health care team members, patients, and families to maximize patient outcomes.
• Collaborates with the team of providers to increase practice knowledge and incorporates that knowledge into the nutrition plan of care.

Qualified candidates will possess:

• Active Virginia Certified Nutrition Specialist (CNS) or Registered Dietitian (RD) License
• Accredited Bachelors or Masters Degree
