- Provides nutritional care for residents in accordance with the facility policies and procedures and consistent with guidelines developed by the New York State Department of Health.
- Assesses nutrition needs and identifies nutrient requirements according to specific needs of residents.
- Accurately and appropriately determines the nutritional status of residents through completion of nutritional assessments.
- Assesses the adequacy of calorie, macro-nutrient, and fluid intake.
- Collaborates with the resident and/or family member, and health care team to assess nutrition and nutrition education needs of patients.
- Identifies need for nutrition supplementation for residents.
- Continuously monitors and evaluates the nutrition care and outcomes of residents.
- Monday-Friday day shift position (Flexible start and end time)
