Job Summary and Qualifications

The Social Worker is responsible for promoting patient-centered care by evaluating psychosocial needs, providing community service coordination for identified patient populations, and providing support and crisis intervention where indicated. The social worker acts as a liaison between the patient and community resources/post-acute care providers. The social worker creates and facilitates timely implementation of discharge plans and arranges post-acute interventions identified in the plan of care. The social worker facilitates patient movement across the continuum of care and identifies, addresses, and tracks barriers to patient throughput.

RESPONSIBILITIES

• Conducts interviews with patient and family members in a therapeutic relationship in order to obtain bio-psychosocial assessment.
• Evaluate psychosocial diagnostic information and provide treatment interventions.
• Provides patient with data on community resources, public assistance and medical care programs administered under Federal, State and County laws and coordinate referrals.
• Assist patient and families through psychosocial evaluations and provide ongoing case management; with special knowledge of growth and development.
• Interpret agency regulations and policies for patient and act as advocate for patients.
• Counsels patient on concerns and makes appropriate recommendations applying developmental theories and age specific comprehension.
• Provide statistical data as needed for hospital studies and/or documentation in a timely manner to Director.
• Act as a hospital resource with regards to public assistance and medical care programs and maintain appropriate reference files.
• Assess educational needs of patient and family by providing instructional and educational materials; conduct workshop, seminars and support groups and serve as a resource.
• Act as a resource regarding advance directive policy for patients.

• Basic Cardiac Life Support
• Masters Degree in Social Work, LCSW Licensure
• 1-3 years experience in social work
• 1 year experience in bariatrics (Preferred)
• Acute care facility experience (Preferred)
