Our agency seeks a compassionate, dedicated mental health professional to join our Clinical team in the Social Worker role. The ideal candidate will possess a Master's Degree in Social Work and be licensed in NYS or plan to obtain license within six months of hire.

The Social Worker will work out of a private office in HCA's inclusive, supportive therapeutic preschool setting. Part of the role may be completed in a remote capacity. Full-time, part-time, and per diem schedules are currently available with tremendous flexibility offered.

This is a NYS Health Care Worker Bonus eligible position.

There is a sign-on bonus of up to $3,000 for this position.

Opportunities and Responsibilities:

• Conduct social history evaluations as a member of HCA Diagnostic & Evaluation Treatment Team for children 3-5 years old.
• Complete ongoing progress reports and recommendations for professional social work services provided for birth-adult population.
• Attend community meetings or hold membership on community committees as deemed appropriate by supervisor.
• Provide Article 16 Clinic counseling
• Document all social work services.
• Complete and maintain paperwork and case records in a timely manner.
• Maintain working knowledge of OPWDD requirements.
• Other duties or programs such as Care Coordination as assigned.
