Summary
The Case Manager is responsible for the care coordination of all residents assigned to their caseload by the Juvenile Mental Health Manager, including but not limited to advocating for resident needs and assisting the Licensed Mental Health Counselors. The Case Manager provides additional support and aids the residents in the completion of their assigned program activities.Responsibilities
? Performs a variety of caseload duties such as serving as a liaison between the Juvenile Detention Facility and assigned probation officers, resident’s families, social service entities, and other related parties.
? Maintains open communication with the Licensed Mental Health Counselors and other staff regarding residents’ needs and issues.
? Prepares, maintains, and submits a variety of records and documentation in a timely manner.
? Provides skills training andr psychoeducation in group and individual settings to residents as directed by Juvenile Mental Health Manager.
? Advocates for the residents’ needs and promotes access to resources, supports, and services in an ethical manner.
? Conducts orientation sessions to explain programs and resources available to residents and engages residents in utilizing these programs and resources as needed.
? Shares program information with residents’ families regarding rules and regulations within Juvenile Detention Facility.
? Performs other duties as assigned..Education and/or Experience
? Bachelor’s degree from an accredited college or university with a major in criminal justice, social,
behavioral, or human service field preferred.
? 1 year experience working with adolescent youth and families.Other Qualifications, Certificates, Licenses, Registrations
? Preference given to individuals who meet state requirements for a Licensed Baccalaureate Social
Worker (LBSW) or a Licensed Professional Counselor Associate (LPC-A).
? Must successfully complete and pass the following clearances:
• Criminal History (TCIC/NCIC)
• Sex Offender Registration Database Check
• Driver’s License
• Pre-employment Drug Screen
