Position Title: Social Worker MSW

**This position is Flexible Days - Fulltime, 9am - 6pm or 10am - 7pm OR PT 24 hours per week**

Overview: The Social Worker provides full range of hospice social work service and bereavement support to patient and families in the care center, home and institutional settings. They work with interdisciplinary team to provide directly, or through coordination of team members, appropriate assessment of and support to patients and families. Also, this position serves as liaison with other community resources; assists staff, patient’s physician and volunteers in understanding significant social and emotional factors affecting a patient and family during terminal illness. The Social Worker provides assessment for normal and complicated grief and provides bereavement counseling as appropriate. Due to business needs, Social Worker will be required to work different shifts, teams and location as needed. If you are compassionate and detail oriented, then we want you!

You Have:
• Master’s in Social Work required from a school accredited by the Council on Social Work Education.
• One (1) to two (2) years of clinical social work experience required. Experience working with interdisciplinary team preferred.
• Ability to embrace individuals from all walks of life.

Location: Gainesville (Provides support to all areas)
