Position Title: Patient Care Manager - Care Center

The Patient Care Manager plans, directs and coordinates the hospice clinical program, inpatient care and nursing home/ACLF program for multiple teams. This position works in an interdisciplinary care setting to teach and mentor patient care personnel. The PCM coordinates all interdisciplinary functions and ensures that patients’ and families’ needs are met and participates in the on-call rotation as necessary. Serves as member of Haven’s management team.

You Have:

• Bachelor’s in Nursing or a related field of Healthcare required. Experience will be considered in lieu of degree.
• Licensure & Certifications- Current Florida RN license without restrictions.
• Requires a current Cardio Pulmonary Resuscitation (CPR) certification (American Heart Association).
• Hospice and Palliative Care Certification preferred. Florida driver’s license and auto insurance required.
• 1 year of managerial experience required and Hospice experience strongly preferred.

The Orange Park Care Center is Haven’s fifth free-standing care center serving north Florida is a state-of-the-art facility that allows us to expand our services to better meet the end-of-life and palliative care needs of patients and families in the area. The care center is located at 745 Blanding Blvd, Orange Park.
