We value quality over quantity and provide you with the time and support to do your best for patients and their families.Job Summary: The LPN provides skilled nursing services to patients in their homes during their end of life in accordance with the physicians’ orders. For this role, you need to be a nurse that is knowledgeable in caring for the terminally ill with the goal of helping patients live their final days with dignity and comfort.

Duties and Responsibilities:
  • Provide direct nursing care procedures to patients
  • Implement an individualized plan of care under the supervision of an RN
  • Prepare documentation on the nursing services that are provided each visit
  • Educate patients and caregivers on coping techniques, end of life care, and provide comfort and support
Qualifications, Knowledge, Skills, and Abilities:
  • Current Missouri Licensed Practical Nurse license in good standing
  • Hospice experience preferred
  • Minimum of one year experience as an LPN within the last three years.
  • Must be flexible with schedule for patients being added for emergency visits
  • Excellent organizational, computer, and time management skills
  • Must be a team player with excellent communication skills
