The Specialty Clinic (BS 5) is located at 79 Retreat Ave. on the fifth and second floor of the Brownstone Building of Hartford Hospital. The clinic has over 4,500 patient visits annually. There are over 10 different specialty clinics held on various days of the week. Clinics include services from Cardiology, Ear Nose and Throat, Endocrinology, Endo Transitional Care, Gastroenterology, Geriatrics, Neurology/Epilepsy, Ophthalmology, Trauma, Transitional Care, and Urology/Cystoscopies. Staffing includes Attending's, PA's, Fellows, Residents, RN's, MA's, MAA's, Central Scheduling and an Office Coordinator. Brownstone staff guides patients through the health system, including appropriate referrals for services to other health professionals.

Job Summary:

HH Referral Coordinator is responsible for new patient referrals being sent to The Specialty Clinic both internally and externally. Patients will be contacted and scheduled according to their location preferences in all Specialty Clinic Service Lines. Schedules will be reviewed daily to review for openings to get patients from the referral Work queue List scheduled sooner to ensure providers have a full schedule. Each referral will be reviewed for completeness, ensure necessary documents are received and the referral meets The Specialty Clinic guidelines for an appointment. The coordinator must properly communicate with front desk staff, providers, clinical lead, and manager. Referral coordinators frequently interact with patients, doctors, nurses, and support staff. The coordinator will be expected to have experience with customer service tasks and the ability to maintain patient records and confidentiality, manage multiple schedules, and ensure that patient information is complete and up to date.


Job Responsibilities:

  • Schedules patients
  • Collaborates and communicates with Specialty Clinic teams to ensure continuity of medical care, including obtaining, clarifying, validating referral requests, and scheduling all referral orders.
  • Assists with the creation of standard work for referral coordination.
  • Responds to all requests (email, website, phone, other) and inquiries to best coordinate a patient's appointment and needs.
  • Maintains referral work queue in a timely manner, updates and closes referrals as appropriate.
  • Contact patients, families, medical providers, and/or facilities to foster smooth transitions of care into the Specialty Clinic.
  • Review referring and transfer documents for accuracy and adherence to regulatory compliance and ensure the transitional care processes are implemented.
  • Consistently communicates with the Nurse Manager to make sure all issues and problems are seamlessly handled so that both the patient and the referral sources are satisfied with the result and process.
  • Responds to internal and external communication timely and accurate.
  • Participates in meetings and performance improvement activities to ensure cost-effective utilization of resources within own control and to provide quality service to the organization through communication, cooperation, and collaboration.
  • Perform other related duties as required

Qualifications

  • High School Diploma Required
  • Minimum of 1 year as a medical receptionist or previous care coordination experience required.
  • Strong written and verbal communication skills
