Our Revisit LPNs utilize their extensiveeducation and training to provide personalized care to our patients in arewarding environment that promotes autonomy. 

· Facilitate communicationbetween the patient and the interdisciplinary team.

· Develop and maintain atherapeutic relationship by providing education and promoting self-advocacy andindependence.

· Utilize state-of-the-arttechnology to document skilled services provided.
