Duties and Responsibilities:

The Direct Support Professional position is responsible for providing hands on direct care and assistance to individuals in all activities relating to daily living skills, personal hygiene skills and social integration in the community. This position shall work under the guidance of and report to the Program Director.

Knowledge, Skills and Abilities:

• Ability to read, write and perform basic math functions.
• Ability to actively assist individuals in performing all activities related to daily living skills, personal hygiene (including toileting), physical and social interactions in the community and other tasks as defined in person’s Individual Service Plan (ISP).
• Ability to actively participate in the activities planned for the individual for each scheduled shift and properly document the reasons for any deviation.
• Ability to assist the individual with performing as many household chores as documented in the ISP. These tasks include cooking, laundry, cleaning, washing dishes, and purchasing groceries.
• Ability to transport individuals to and from their community functions, appointments, church, etc. and adhere to all driving rules while doing so.
• Ability to accept direction and feedback from the Program Director.
• Ability to document the activities of the individual including any related observations about the individual in their progress notes; Date and sign all entries properly.
• Ability to strictly follow agency policy and procedure regarding the appropriate method of amending (adding, changing, deleting or correcting) individual documentation.
• Ability to maintain a safe, sanitary and therapeutic environment for the individuals being served.
• Ability to maintain a professional attitude.
• Ability to complete incident reports prior to the end of the shift in which each incident occurred, to the best of your ability for those activities and observations that you directly witness and/or were responsible. All incident reports are to be completed and submitted to the Program Director at the end of the shift.
• Report, according to policy, all cases of suspected abuse and/or neglect.
• Interact with individuals, their family members and other interested parties, in a courteous, respectful and professional manner.
• Attend all meetings and trainings as instructed by the Program Director.
• Maintain valid driver’s license, auto insurance, CPR/First Aid Certifications, Medicaid Waiver Certification, TOVA certification and all other certifications as required by Licensing or best practice.
• Complete all other assignments as delegated by the Program Director in support of individuals being supported by the agency.
• Ability to complete special projects and other duties as assigned.

Experience:

Education equivalent to graduation from high school. One year of full time or equivalent part time or volunteer experience in the care, training, habilitation and/or development of working with individuals with intellectual, developmental, mental health, physical and/or other disabilities. Successfully complete Criminal History and Central Registry background investigations. Must possess a valid driver’s license and automobile liability insurance.

As a condition of employment/contractual agreement, Harmony1, LLC staff/contractors will:

3. Protect individuals from any form of abuse, neglect and exploitation by not

abusing, neglecting or exploiting any individual; by not permitting or condoning anyone else to abuse, neglect or exploit any individual and by reporting all suspected abuse to his/her immediate Supervisor and the Agency Human Rights Advocate.

4. Cooperates with any investigation, meeting, hearing or appeal held under these Policies. Cooperation includes but is not limited to, giving statements or sworn testimony.

5. Perform any other duties required.

6. Maintain strict confidentiality.

Pay: $15.00 per hour
