Under the direction of the Administrator:
- Coordinates and directs Person Directed Care (PDC) programs through education of employees, residents, families, community members, and healthcare providers
- Provides support and direction for teams to achieve the mission and vision of culture change
- Coordinates efforts to achieve Person Directed Care (PDC) benchmarks, as part of the ABCM Person Directed Care Certification program
Qualifications:
- High school diploma or equivalent
- Must possess outstanding communication skills
- Experience in leading and motivating team members preferred
- COVID-19 vaccination required
