Description

The Licensed Practical Nurse (LPN) is a primary provider of resident care. The LPN promotes the physical, personal, and emotional well-being of each resident and strives to maintain the residents at their maximum level of self-care and independence. The ultimate goal of quality resident care is to enable each resident, where possible, to return to the highest level of functioning, to attain a new stage of independence, or to at least prevent regression. Resident care is not passive maintenance; an active approach will always be taken.

Harbor’s Edge is a premier resort-style Continuing Care Retirement Community (CCRC) located on the harbor in Norfolk, Virginia. Harbor’s Edge is nearing completion on an expansion that includes an additional 24-story tower. When complete, Harbor’s Edge will have 300 residential units in addition to a 33 bed (Medicare/Private Pay) Skilled Nursing unit and 80 Assisted Living/Memory Care units. The Community will employ approximately 300 employees.

Responsibilities

  • Provides direct nursing care services to residents and follows resident care plan for all residents (ADL Assignment Worksheets)
  • Performs resident assessments as required by regulations and facility policy under the direction of the Director of Nursing (DON) and scope of practice
  • Observes residents for change in condition and reports any changes immediately to nursing supervisor and/or the Director of Nursing as well as resident’s physician as directed
  • Documents all pertinent/relevant observations, communication with the resident’s physician & responsible party, clinical procedures, and daily assistance/care by the end of each shift
  • Updates, completes, and maintains plan of care for each resident
  • Attends all scheduled interdisciplinary resident care meetings
  • Checks and records vital signs, weights, and blood sugars as assigned
  • Administers medications, injections, and other treatments as authorized by current licensure by the VA State Board of Nursing
  • Provides for or assists patients with activities of daily living as needed, including bathing/showering according to schedule, oral care, toileting, dressing, etc.
  • Performs restorative nursing techniques including proper positioning, ADL training, and Passive Range of Motion
  • Assesses resident and ensures complete documentation of skilled need
  • Assesses safety of all medication, including narcotics, with inter-shift count
  • Encourages residents to participate in recreational activities and assists residents in getting to activities
  • Assists in the planning and implementation of resident activities as needed
  • Assists residents with transfers and ambulation as directed and ordered by the physician using proper body mechanics; use of facility gait belts, back supports, and mechanical devices is required
  • Responds to urgent calls as needed
  • Maintains restraint free environment at all times and supports fall prevention programs for all residents in need
  • Helps admit and discharge residents according to facility procedure
  • Assists in post-mortem care as instructed
  • Orients new employees
  • Assists in preparation of monthly Community newsletter
  • Assists with planning, organizing, and functioning of various resident committees
  • Attends in-service classes and staff meetings
  • Performs other duties as assigned

Requirements

  • High school diploma/GED at a minimum
  • Current state license as an LPN in the Commonwealth of Virginia or Compact State
  • Minimum of one year of experience at this level of responsibility
  • Minimum of one year of experience in a skilled nursing care facility or other related medical facility
  • Excellent verbal and written skills
  • An up-beat and engaging personality
  • Able to follow spoken and written instructions
  • Must have a desire to work in a Continuing Care Retirement Community (CCRC)
  • Must be able to deal tactfully with personnel, residents, family members, visitors, and the general public
  • Must have a heartfelt compassion for this line of work
  • Must be able to exert 50-100 lbs of force to assist elderly in and out of wheelchairs
