NOTE: The following position is for PER DIEM (per patient / assignment) basis. We are looking for Home Health experienced visiting nurses who are willing to be processed for **POTENTIAL** cases in the future. It is based on case availability. Please do not apply if you have part time/full time expectations.

ADDITIONAL NOTE: Our office is located in WEST COVINA, however we service patients all throughout Southern California. Please provide your service areas as applicable.

HOME HEALTH RN (PER DIEM) job description:

• Registered Nurses (RNs) are responsible for organizing and directing home care services. They conduct assessments to determine home care needs. They will initiate and supervise a plan of care for their assigned patient. They report to the case manager/Clinical supervisor as needed and at times, the Physician directly.
• RNs are also expected to maintain and report accurate and detailed charting/documentation of their patient visits.
• The job details involve HOME HEALTH PATIENTS and rendering care at the PATIENT'S HOME.

REQUIRED HOME HEALTH EXPERIENCE:

• Must have a valid & up-to-date license as a Registered Nurse (RN).
• Must have 1 year minimum of experience in a healthcare setting, preferably in primary care or a similar setting.
• Must have & maintain current CPR/BLS certification.
• Must be a licensed driver with auto insurance.
• Must have good reporting and documenting (charting) practices. Preferred experience with our electronic medical record (EMR) system: DSL or 'UltraHHC' but not a deal-breaker.
• Must have HOME HEALTH experience due to the position requiring Home Health Assessments and Start of Care visits (SOC). This involves experience with charting OASIS.

Pay: From $55.00 per hour
