Harbor Health also operates two Programs for All-Inclusive Care for the Elderly in Mattapan and Brockton, providing comprehensive healthcare, transportation and social services for more than 580 frail elders who continue to live with dignity and independence in the community and a Woman, Infants and Children (WIC) Nutrition Program. To help further the mission of Harbor, we are looking for a talented Nurse Practitioner for the Medication Assisted Treatment Program (MAT) to join our teams at the Harbor Community Health Centers located in Plymouth and Hyannis, MA

Role: 

Under the direction of the Director of the MAT Program (Medication for Addiction Treatment), the Nurse Practitioner for the MAT Program provides appropriate care to patients with a focus on Addiction Medicine in our established MAT Program. The Nurse Practitioner also provides management of acute illnesses providing Same Day Visits. This position would require travel between sites in Plymouth and Hyannis, MA for clinical sessions.

Responsibilities: 

  • Manages patients with SUD (Substance Use Disorder) using a streamlined workflow that focuses on a patient’s recovery journey and dedicated MAT appointments.
  • Prescribes Buprenorphine as well as Naltrexone (including administration of extended-release Naltrexone/Vivitrol) in accordance with ASAM/SAMHSA guidelines.
  • Coordinates care with Behavioral Health as well as Primary Care Providers utilizing EMR and multidisciplinary care team meetings and other division meetings as appropriate.
  • Works as part of a team with Director and other MAT providers in providing backup coverage to support the team and patient care.
  • Provides same day access, walk-in and patient overflow care at the health center(s). 
  • Provides support to the Primary Care Providers in the care of primary care patients, including outpatient visits, communication with patients by mail, email or phone, inpatient care, referral for specialty care, provision of or referral for patient education for self-management, assesses health risks, promotes and provides disease prevention, and practices evidenced based medicine.
  • Supports the Primary Care providers with routine health maintenance of primary care patients which may include a complete history and physical, developmental assessment, risk assessment, cancer screening, and health promotion education. Follows up on any problems detected. Makes appropriate referrals for additional services required, including, nutrition, social services, behavioral health, specialty care.
  • Provides timely and expert care to patients with acute illness.
  • Manages common chronic illnesses, including acute complications and routine follow-up, provides or arranges for appropriate patient education regarding self-management of chronic illnesses.
  • Participates in an on-call system with other providers in the Center and with other providers at affiliated health centers that provides 24-hour access to a provider for our patients. 
  • Participates in evening/weekend/holiday clinic schedule to provide face to face care, follow up visits, acute care and other clinical services to patients.
  • Participates in provider staff meetings, departmental meetings, general staff meetings, trainings, care team meetings and quality improvement/practice transformation sessions. 
  • Complies with state continuing education laws. Enhances own clinical practice by attendance at formal and informal educational programs.
  • Participates in managed care plans and assists the Center by reinforcing patient understanding of such plans; makes appropriate, cost-efficient referrals and admission decisions and refers only to participating specialists; provides guidance and monitoring of systems necessary for the function of managed care programs.
  • Updates Electronic Medical Record (EMR) appropriately using proper coding and documents care accordingly; accurately and timely documents care in the medical record with progress notes, lab follow-ups, communications with patients; maintains a current problem list, medication flow sheet, immunization record and other appropriate information.

Requirements:

  • Completed accredited Nurse Practitioner program in Family Practice or Adult Medicine, Board Certified
  • Family and Addiction Medicine experience required 
  • Current MA RN/NP license, Board Certified, current DEA and MCSR required
  • Current BLS (Basic Life Support for Healthcare Providers) Certification required
  • Experience with a community health center population and practice setting preferred 
  • Computer Literate (MS Office 365) and EMR (EPIC/OCHIN highly desired)
  • Must be able to travel between Harbor sites in a timely manner
