Job Summary:
Hands Of Healing’s Case Manager will ensure the safe and timely release of UC within ORR timeframes. Within twenty-four (24) hours of placement, the CM will conduct and provide UC with a program orientation. The CM will work collaboratively with the designated Clinician to address the immediate needs of the UC through the development of the UC Assessments. The CM will work with the designated Clinician to develop safety plans for UC when: a history of abuse is identified; there is specific need identified (medical, physical, or behavioral), or when situations involving non-routine events take place that involve UC in care. The CMs plan for the safe and timely release of UC by gathering required information, verifying family relationships, requesting supporting documentation, and screening for risk and exploitation. Within twenty-four (24) hours of the identified sponsor, the CM will send the sponsor the FRP (family reunification packet).
They are also responsible for the submission of service plans and other assessments required for the compiling of a comprehensive case file as per HOH’s policies and protocols. The Case Managers are required to maintain a flexible, organized, and efficient work schedule and is subject to work extended hours, weekends, and be on-call.

Must be able to obtain and perform Emergency Behavior Intervention Training (EBI)/Crisis Prevention Institute (CPI) certification and First Aid Certification (CPR). Must be able to supervise clients indoors and outdoors as necessary. Bending, stooping, and lifting up to 20 lbs. required to complete daily tasks. Job requires extended periods of sitting and use of various office equipment.
Work Environment:
Work is at a youth care facility at which minors receive multiple services. Two shifts per twenty-four-hour period. Work shifts are subject to change. Noise levels vary from mild to moderate. Predominantly indoors, occasional outdoor monitoring or activities.
