The ideal candidate is a self-starter, can manage their own schedule, is dependable, responsive to the needs of patients, communicates well with the team, and can show compassion and empathy in all interactions with patients and their loved ones.

This is a full-time, Monday through Friday, dayshift position - enjoy your evenings and weekends off! The social worker will collaborate with the interdisciplinary team to create and implement care plans that meet patients and their loved ones' physical, spiritual, emotional, and psychosocial needs.

The Social Worker is responsible for the social and emotional needs of patients and their loved ones. The social worker provides counseling, facilitates conversations regarding advance directives and other end-of-life decisions, connects families to community resources, provides education, and advocates for patients’ wishes. The Social Worker is an interdisciplinary team member who works collaboratively with the Medical Director, RN Case Manager, and Spiritual Care Coordinator to create individual care plans.

The Social Worker has autonomy over scheduled visits and travels locally to care for patients in their house, assisted living, or nursing facility.

Local travel to nursing facilities and the home setting is necessary. You will be compensated for this travel with a generous car allowance.

RESPONSIBILITIES

  • Conduct comprehensive psychosocial assessments: Perform in-depth assessments of patients and their families to evaluate their emotional, social, and practical needs. Gather relevant information to understand the patient's support system, cultural background, and any psychosocial factors that may impact their care.
  • Provide counseling and emotional support: Offer empathetic and compassionate counseling to patients and their families, addressing their emotional and psychological concerns related to end-of-life care. Help individuals cope with grief, loss, and other challenging emotions, promoting their overall well-being.
  • Develop and implement care plans: Collaborate with the interdisciplinary team to develop individualized care plans that address the psychosocial needs of patients. Ensure care plans align with patient preferences, cultural beliefs, and ethical considerations, promoting a holistic approach to care.
  • Assist with advance care planning: Facilitate conversations and documentation regarding advance directives, durable power of attorney for healthcare, and other end-of-life decision-making processes. Support patients and families in expressing their wishes and ensuring their preferences are respected.
  • Coordinate community resources: Identify and connect patients and families with appropriate community resources and support services, such as counseling services, support groups, financial assistance programs, and home health services. Assist with referrals and coordinate the necessary paperwork and documentation.
  • Provide information and education: Offer education and information to patients and families about hospice services, end-of-life care options, and available community resources. Ensure individuals are empowered to make informed decisions and have a clear understanding of the support available to them.
  • Advocate for patient and family needs: Serve as an advocate for patients and families, ensuring their concerns, preferences, and rights are respected and addressed. Collaborate with the healthcare team to address any barriers to care and facilitate effective communication between patients, families, and healthcare providers.
  • Collaborate with interdisciplinary team: Work closely with the interdisciplinary healthcare team, including physicians, nurses, chaplains, and other professionals involved in the patient's care. Participate in team meetings, case conferences, and care planning sessions to share insights, contribute to the overall care plan, and ensure coordinated care.
  • Maintain documentation and compliance: Keep accurate and timely documentation of patient assessments, interventions, progress notes, and care coordination activities. Adhere to regulatory and organizational standards for documentation, privacy, and confidentiality.

REQUIREMENTS

  • MSW
  • Minimum of one-year Hospice experience is preferred.
  • Valid driver's license and reliable transportation that is insured in accordance with state and/or Hand in Hand requirements.
  • Demonstrate excellent observation, assessment, judgment, and communication skills. Ability to provide accurate written documentation by the end of shift.
  • Self-directed with the ability to work with little supervision.
