Hamaspik Care is Hiring an Intake Coordinator-Bilingual in Spanish Near Spring, NY

Growing Home Care Agency is seeking a Part-Time Intake Coordinator to join our dynamic team at our Spring Valley office.Responsibilities include but are not limited to:  
  • Serve as initial point of contact for potential clients and their families, responding to inquiries and providing information about the company’s services, policies, and procedures.
  • Conduct comprehensive assessment of clients’ needs, including physical, emotional, and social requirements, to determine the appropriate level of care and support.
  • Gather necessary information during the intake process, including medical history, current medications, allergies, and any specific care preferences and requirements.
  • Collaborate with clients, their families, and nursing team staff to develop personalized care plans that alight with the client’s goals and needs.
  • Assist current clients with applying for additional services when eligible.
  • Make referrals to other social services agencies or community organizations that can assist when clients are not eligible for Hamaspik services or may require assistance beyond the scope of what Hamaspik offers.
  • Maintain accurate and up-to-date client files, including insurance information, consent forms, and other relevant documentation.
  • Establish and maintain positive and professional relationships with clients, their families, and other stakeholders, demonstrating empathy, active listening, and effective communication skills.
  • Ensure compliance with the regulatory requirements and company policies related to client records and confidentiality of information.
  • Participate in regular meetings and training sessions to stay updated on industry trends, best practices, and changes in regulations.
  • Other duties as assigned.
Qualifications
  • Excellent verbal and written communication skills to interact effectively with patients, families, and healthcare professionals
  • Strong organizational abilities to manage multiple tasks, prioritize, and maintain accurate records.
  • Ability to build rapport with patients and their families, demonstrating empathy and understanding.
  • Meticulous attention to detail to ensure accuracy in patient information and documentation.
  • Strong problem-solving abilities to address and resolve patient concerns and coordinate care effectively.
  • Efficient time management skills to handle a high volume of intake assessments and follow-ups.
  • Proficient computer skills.
  • Bilingual in Spanish
