We are searching for an energetic Home Health Aide to join our team full time. Enjoy working one on one with your patients in the most functional environment possible --- their home. The ideal candidate will be patient care focused, caring, compassionate, empathetic, organized, flexible, comfortable using a computer, and possess excellent communication & customer service skills.

Responsibilities:

· Tasks performed by a Home Health Aide must be assigned and directed under the supervision of a registered nurse.

· Assist clients with daily personal healthcare, such as bathing, dressing, grooming, and other activities of daily living.

· Encourages the client to become as independent as possible according to the nursing care plan.

· Assists the client with ambulation and assists the therapy personnel as needed with the rehabilitative processes.

· Attempts to promote client’s mental alertness through engaging in appropriate conversation and involvement in activities of interest.

· Gives simple emotional and psychological support to the client and family members.

· Documents care services rendered on daily progress notes and submits them to the Agency weekly.

· Reports any change in the client’s condition or in his or her home situation to the staff nurse or Director of Nursing

Organizational Relationship:

· Reports directly to the staff nurse and indirectly to the Director of Nursing.

Functional Abilities:

· Must be able to communicate verbally and in writing in a manner understood by most persons.

· Must be able to lift and transfer clients.

· Must be able to travel to client’s residences.

Special Requirements:

· Must have a reliable car, current car insurance and driver’s license.

· Must be able to successfully pass a background check.

· HHA or STNA certification is preferred; training is available.

Home Health Aide Certification OR STNA (Preferred but not Required)
