Community Health Worker 

(Bilingual Haitian-Creole)

PURPOSE OF POSITION
The Community Health Worker (CHW) is responsible for providing navigation and retention services for PLWHA at all stages of care, including treatment, prevention, and essential support services. They will establish collaborative relationships with government agencies, other non-profit organizations, and community groups, offering support and guidance to improve the overall health outcomes of the participant.
RESPONSIBILITIES
  • Accompaniment: Accompanies participants to HIV primary care and supportive service appointments if needed by the participant. Accompaniment can be initiated from the participants’ home, office, or other mutually agreeable location, including the site of the appointment.
  • Care Coordination: Facilitate and provide care coordination services for participants through face- to-face interactions, or telephone conference when the participant’s participation is required. Manage and facilitate any required tasks to connect the participant to needed services. This may include phone calls with the participants’ medical provider or with other service providers. 
  • Referrals: Serve as a bridge between the participants and providers by facilitating referrals to medical
    professionals, clinics, and social services agencies. Advocate for patients' rights and ensure they receive
    appropriate care within various social and healthcare systems. Coordinate appointments and follow-up
    care for individuals requiring additional treatment or services.
  • Case Conferences: Conduct and/or participate in case conferences with other internal or external providers (participant is not required to be present for case conference).
  •  Prevention Services: Provide services directly to and/or on behalf of the participant to facilitate access to a variety of services including primary medical care, social services, housing entitlements and benefits.
  • Participant Documentation: Assist participant with any necessary paperwork. Compile eligibility documentation required by other service providers.
  • Health Assessment and Monitoring: Monitor the participants to ensure they are adhering to treatment plans and receiving necessary support. Responsible for Service Plan Development, Reassessments, referral to legal services, substance abuse, and mental health services as needed. Keep detailed records of health assessments and observations for reporting and documentation purposes
  • Health Education and Promotion: Educate participants about preventive healthcare practices, such as nutrition, hygiene, etc. Organize workshops or meetings to raise awareness about specific health issues prevalent in the community. Provide educational materials and resources to disseminate important health information. Facilitate group meetings and provide HIV/STD risk reduction counseling and referrals.
  • Re-engagement Outreach: Provide ongoing support to encourage retention in services. Re- engagement activities should be conducted via a series of phone calls and written correspondence, and/or in-person visit(s) to the participant’s place of residence. 
  • Data Collection and Reporting: Collect data on community health trends, demographics, and health behaviors to inform program planning and evaluation. Compile reports on program activities, outcomes, and challenges for stakeholders and funding agencies. Participate in quality improvement initiatives to enhance the effectiveness of community health interventions. Submit participant-level data entry, and lab retrievals (Quarterly) via E-share.
  • Other duties and responsibilities may be assigned as necessary.
EDUCATION
  • High School Diploma or GED, and at least two years of applicable experience and/or equivalent experience.
  • Associates Degree or higher in Human Services or other relevant degree preferred.
QUALIFICATIONS:
  •  Fluency in both English and Haitian-Creole required.
  •  Excellent time management and prioritization skills.
  •  Must be detail-oriented.
  • Must maintain confidentiality regarding agency and participant information.
  • Motivated team player with great interpersonal skills
  • Must possess a strong desire to support the team in meeting organizational objectives.
  • Ability to identify and resolve problems/queries quickly and efficiently.
  • Effective written/verbal communication.
  • The CHW will work in a variety of settings, including community centers, clinics, and homes. Travel within the community/local region to be expected.
Compensation & Benefits**
· The base pay range for this position is $40,000-$50,000 per year. *
