SUMMARY

• High school diploma or GED preferred; but not required.
• Must have the ability to independently travel within the targeted communities. Access to automobile is preferred.
• Excellent interpersonal skills and ability to interact with a wide range of culturally diverse individuals experiencing crisis and distress.
• Ability to think critically and communicate both verbally and in writing in a professional manner.
• Must possess basic computer skills and knowledge of office software programs.
• Must be 18 years of age or older.
• Must reside or experience working in the targeted community or in the high economic hardship communities.

• Engage with clients to identify challenges they face and facilitate referrals.
• Provide clients with information and guidance related to ongoing services.
• Comply with training regarding confidentiality and handling of personal identifiable information; follow all scripts policies and procedures established for the program.
• Collaborate with supervisor to identify project goals collect and record information regarding individuals needs goals referrals outcomes and other relevant data.
• Participate in ongoing professional development and related training as well as scheduled team meetings.
